Abstract
This paper explores the formulation and implementation of healthcare policy in China through case studies focused on the medical delivery system reforms initiated by the government since 2010. The findings indicate that the central government has categorized healthcare policies into several contents and promptly enacts policies where clear policy tools are available, such as the training of family medicine specialists. In contrast, for referral systems where policy tools are less defined, the government has relied on policy experiments to identify effective approaches. During this process, rural areas—where government influence is particularly strong—have actively implemented policy experiments, although there were some regional variations in policy models. However, since medical institutions are the primary agents responsible for carrying out these policies, large urban hospitals that generate significant revenue from medical services have often failed to fully implement government directives. As a result, policy experiments are still ongoing. In other words, when policies promoted through policy experiments are implemented by independent organizations rather than the government, there is a risk that these initiatives may remain in a perpetual experimental phase.
