성인의 연명치료에 대한 태도

본 연구의 목적은 Q-방법론을 이용하여 성인의 연명치료에 대한 태도의 주관성 구조를 파악함으로써 연명치료에 대한 사회적 논의를 위한 근거 자료를 제공하고, 생의 마지막 과정 동안 환자와 가족의 삶의 질을 증진할 수 있는 간호전략개발의 기초 자료를 제공하고자 함에 있다. 연구 결과 나타난 성인의 연명 치료에 대한 태도는 4개의 유형 - 합리적 가치 존중형, 순리 중시형, 생명 중시형, 종교 중시형 등을 발견하였다. 연구대상자들의 연명치료에 대한 공통적 견해는 사람은 품위 있게 죽음을 맞이해야 할 권리가 있으며 연명치료는 자신의 가치관을 중심으로, 살아있을 때 사전에 의사결정을 하는 것이 바람직하다는 생각을 나타내었다. 본 연구 결과를 토대로 연명치료중지에 대한 사회적 합의점을 도출하고 무의미한 생명 연장으로 인한 고통 대신 존엄한 죽음을 맞이하도록 환자의 삶의 질을 고양하는 방안과 정책개발을 논의하였다.
Abstract
Objectives: The purpose of this study was to identify on the attitudes toward the life sustaining treatment (LST) among Korean adults according to their values, beliefs, and attitudes using Q methodology. Methods: Forty-two Korean Adults classified 33 selected Q-statements into 7 points standard. The obtained data were analyzed by using a QUANL-pc program. Results: Principal Component Analysis identified 4 types of the attitudes toward the life sustaining treatment among adults, and named by researcher. Type Ⅰ is ‘Respect for reasonable value’, type Ⅱ is ‘Emphasis on rationality’, type III is ‘Emphasis on life’, and type IV is ‘Religious focused type’. The results suggest that attitudes toward the life sustaining treatment among Korean adults should be understood for dignity and well-dying in Korean context. Conclusion: Based on these results, the directions for future research were discussed.
