노인요양시설의 감염관리 영향요인 분석

An Analysis on the Influencing Factors of Infection Control in Elderly Care Facilities

본 연구는 우리사회의 노인요양시설 감염관리 증진에 필요한 영향요인 규명에 목적을 두었다. 이를 위해 감염관리를 태도와 행동으로 구분하고, 감염예방과 통제계획, 감염관리자 지정, 감염관리 모니터링, 감염교육, 협력적 조직문화를 영향요인으로 설정하였다. 전국에서 노인요양시설이 가장 많이 밀집되어 우리사회의 시설 감염관리 특성을 가장 잘 반영할 것으로 평가되는 고양시의 83개 시설 536명을 대상으로 한 분석결과, ① 시설차원의 감염관리 노력에 비해 구성원의 감염관리 관심과 개선의지가 미약했다. 이러한 성향은 시설규모가 클수록 뚜렷했다. ② 감염관리에 미치는 영향력은 협력적 조직문화, 감염관리 모니터링, 감염교육, 감염관리자 지정 순으로 나타났다. ③ 감염관리 영향요인별 특성에 있어 협력적 조직문화는 감염관리 태도개선에 가장 영향이 크고, 감염관리 실천도 증진에는 감염관리 모니터링의 영향이 가장 크게 나타났다. 감염교육은 관심 증가와 실천도 개선에는 영향이 크나 개선의지 향상에는 영향이 없고, 감염관리자 지정은 태도변화에는 도움이 되나 행동변화에는 영향이 없었다. 감염예방과 통제계획은 감염관리와 상관성은 있으나 인과성은 없었다. 이러한 결과를 토대로 본 연구는 노인요양시설의 감염관리 취약 원인 진단과 함께 이를 보완할 향후 과제에 대해 논의하였다.

Abstract

This study aimed to identify the influencing factors for the promotion of infection control in elderly care facilities. To this end, infection control was divided into attitudes and actions, and plan of infection prevention and control, infection preventionist, infection control monitoring, infection education, and cooperative organizational culture were set up as the influencing factors. Based on the 83 elderly care facilities in Goyang City, the results showed that ① The members' interest and willingness to improve infection control were weak compared to the efforts of infection control in facility level. This tendency was evident as the size of the facility was larger. ② Influencing factors’ Influences on infection control were shown in the order of cooperative organizational culture, infection control monitoring, infection education, and infection preventionist. ③ In terms of the characteristics of influencing factors, cooperative organizational culture was the greatest impact on improving the attitude of infection control, and infection control monitoring was the greatest impact on the promotion of infection control practices. Infection education was a large impact on increasing interest and improving practice, but it was not affect willingness to improve infection control. Designation of infection preventionist helped attitude change, but it was not affect behavioral change. The plan of infection prevention and control was related to infection control but not causative. Based on these results, this study discussed future tasks to supplement the diagnosis of the causes of infection control in elderly care facilities.
