노인요양시설의 감염관리 체계와 역량 변화 분석: 코로나 전･후 비교를 중심으로

본 연구는 코로나를 계기로 변화되기 시작한 노인요양시설의 감염관리 체계개선 노력이 코로나 이전 대비 어떤 성과가 있었는지를 분석하는데 목적을 두었다. 이를 위해 선진 6개국 감염관리 체계에 의거 2018년과 2023년에 조사된 두차례의 설문자료를 바탕으로, 코로나 전후의 우리사회 감염관리 체계, 감염관리 역량, 감염관리 인식과 실천도를 비교하였다. 분석결과, ① 전반적으로 감염관리 역량과 실천도는 개선되었으나 감염관리 체계 개선은 여전히 미흡한 것으로 나타났다. ② 평가항목이 개정된 영역은 그렇지않은 영역 대비 감염관리 역량과 실천도 개선이 높았다. ③ 전반적인 감염관리 개선에도불구하고 감염관리 인식은 오히려 줄어든 것으로 나타났다. 코로나 이전 대비 감염관리관심과 개선의지 모두 감소했고, 특히 이러한 모습은 노인공동생활가정보다는 요양시설, 요양시설 중에서는 30인 이상 대형 시설일수록 뚜렷했다. 이러한 결과는 코로나를거치면서 시설 내 감염관리 중요성에 대한 공감대 형성으로 코로나 이전 대비 감염관리개선은 있었으나, 감염관리 체계 개선 미흡에 따른 유인동기 부족으로 현재의 성과가 지속성 없는 한시적 성과일 수 있음을 시사한다. 이를 토대로 본 연구는 우리사회 노인요양시설의 감염관리 개선을 위한 몇 가지 과제와 그 실천방안에 대해 논의하였다.
Abstract
The purpose of this study was to analyze the results of efforts to improve the infection control system of elderly care facilities, which began to change in the wake of the corona pandemic, compared to before the corona pandemic. To this end, based on the data of two surveys conducted in 2018 and 2023 according to the infection control systems of six developed countries, I compared the infection control system, infection control capabilities, and awareness and practice of infection control in our society before and after the corona pandemic. As a result of the analysis, it was found that (1) overall infection control capabilities and practices have improved, but the improvement of the infection control system is still insufficient. (2) In areas where the evaluation items were revised, infection control capabilities and practices were also improved compared to those where they were not. (3) Despite the improvement in overall infection control, awareness of infection control has decreased. Compared to the pre-corona period, both interest in infection control and willingness to improve decreased in elderly care facilities, and this was especially evident in large facilities with more than 30 people in elderly care facilities and elderly care facilities rather than in elderly co-living homes. These results suggest that although there has been an improvement in infection control compared to before Corona due to the formation of a consensus on the importance of infection control in facilities during the Corona period, the current performance may be a temporary achievement without sustainability due to the lack of incentive due to the lack of improvement of the infection control system. Based on these results, this study discussed some issues and practical measures to improve infection control in elderly care facilities in our society.
