Quantitative light-induced fluorescence (QLF) is a dental diagnostic tool that utilizes the natural autofluorescence of teeth to detect early carious lesions. This study aimed to evaluate the reliability of maximum fluorescence loss (ΔFmax), mean fluorescence loss (Fmean), and maximum red fluorescence (ΔRmax) on multiple QLF images of a single carious lesion and to analyze their correlation with each other and with carious cavity depth. Nineteen patients (29 teeth) with caries in the cervical regions of anterior and premolar teeth were included. Each lesion was imaged three times using the Qraypen C (All-in-one Bio, Seoul, Korea) before caries removal. Images were analyzed using Q-ray software to obtain ΔFmax, Fmean, and ΔRmax. Intraclass correlation coefficients (ICC) were used to assess reliability, and Spearman’s ρ to analyze variable relationships. Statistical analyses were conducted using R version 4.1.2. Overall ICC values indicated high reliability: 0.849 (ΔFmax), 0.914 (Fmean), and 0.917 (ΔRmax). Buccal lesions showed higher ICCs (ΔFmax: 0.872, Fmean: 0.912, ΔRmax: 0.933) than mesial/distal lesions (ΔFmax: 0.679, Fmean: 0.829, ΔRmax: 0.895). A positive correlation was found between the absolute values of ΔFmax, Fmean, and ΔRmax. However, no significant correlation was observed between cavity depth and any fluorescence variable. Repeated QLF imaging of a single lesion demonstrated high reliability for ΔFmax, Fmean, and ΔRmax. A positive correlation was observed between fluorescence loss and red fluorescence, while cavity depth showed no significant association with QLF metrics.
