Abstract
Non-alcoholic fatty liver disease (NAFLD) is a growing metabolic disorder in children and adolescents that is strongly linked to obesity, insulin resistance, and metabolic syndrome. This study examined the key pathophysiological mechanisms, risk factors, and the impact of exercise and physical activity on prevention and treatment. NAFLD is associated with insulin resistance, inflammation, mitochondrial dysfunction, hepatokine dysregulation, and genetic and environmental factors that influence disease progression. These findings indicate that aerobics, resistance, and combined exercises reduce liver fat accumulation, improve insulin sensitivity, and lower NAFLD risk in active individuals. Additionally, exercise modulates hepatokine expression related to liver metabolism and fatty acid oxidation. Exercise and physical activity are effective non-pharmacological interventions that play crucial roles in NAFLD prevention and management. Therefore, it is essential to promote sustained physical activity and develop tailored exercise protocols. Future studies should explore the effects of different exercise modalities and intensities on hepatokine regulation and on long-term health outcomes.
