일반적으로 한국의 노인환자는 의료적 문제 및 요양에 대한 니즈를 해결하기 위해 요양병원을 이용한다. 인구 고령화에 따라 만성질환을 가진 노인 환자와 요양병원의 수는 지속적으로 증가하고, 노인의료비 또한 증가하게 될 것이다. 고령화 사회로 진입할수록 노인의 건강권 보호를 위한 요양병원의 적극적인 기능과 역할이 강조되는 것은 당연한 것이다. 요양병원에 대한 사회적 관심이 집중되고 있음에도 불구하고 요양병원에 대한 다양한 문제점이 지적되고 있다. 즉 시설의 불안전성, 전문적인 서비스 제공 인력의 부족, 과잉 서비스 제공에 따른 부당이득, 노인에 대한 인권침해 등의 문제점들이 반복적으로 노출되고 있는 실정이다. 이는 노인의 건강권뿐만 아니라 국민건강보험 재정의 건전성을 위협하는 것이다. 이러한 문제를 해결하기 위해서는 요양병원에 대한 정확한 실태조사 및 엄격한 관리감독은 물론, 보건의료체계 내에서 요양병원의 기능 재정립, 요양병원의 시설 및 인력 확보, 노인 인권 보호 방안 등이 종합적으로 논의되어야 한다. 노인의 건강권 보호를 위해 세계 각국은 노인에 특화된 의료서비스 및 보험제도를 개발·운영하고 있다. 전국민건강보험제도 내에서 요양병원제도를 별도로 운영하는 국가는 극히 드물다. 요양병원을 별도로 운영함으로써 노인에 대한 의료서비스를 강화할 수 있는 것은 외국과 비교하여 우리나라의 강점이라 할 수 있다. 세계적으로 유례없는 급속한 고령화가 진행되고 있는 가운데, 요양병원이 보건의료체계 내에서 노인의 건강권 보호 기능을 적절하게 수행한다면, 우리나라 노인의 삶의 질 향상에 크게 기여하게 될 것이다.
Abstract
As we move toward an aged society, both the number of elderly patients with chronic disease and long-term care hospitals are increasing. The medical expenditures for the elderly will increase accordingly. It stands to reason that strengthening the role and function of the long-term care hospitals to protect the health rights of the elderly as countries move towards an aged society. Together with the growing interest in long-term care issues in society, various related disputes have been raised, such as the instability of facilities, lack of the professional personnel, unfair profit by providing unnecessary services, and infringement of the human rights, etc. These not only threaten the health rights of the elderly but also the national health insurance finance as well. In order to resolve these problems, strict management and supervision on long-term care hospitals are essential. Thus establishing plans for securing facilities and personnel of the long-term care hospitals and protecting the human rights of the elderly must be discussed comprehensively. There are few nations operating long-term care hospitals under the national health insurance system. Korea operates long-term care hospitals aimed at the elderly care apart from other traditional hospitals. This is the strength that our country has. Facing an unprecedented pace of population aging in the world, if long-term hospitals perform their function properly under the health care system, they will make significant contribution to the improvement in the quality of life of the elderly in Korea.
