본 연구는 프랑스의 노인 정책 발전 과정과 장기요양제도의 도입 배경을 살펴보고, 제도 시행 과정에서 APA의 개념이 어떻게 해석되고 적용되는지를 분석하고 있다. 또한 APA의 재정 및 운영 권한이 지방 정부로 이양됨으로써 이 정책이 유연성과 서비스의 접근성을 확대할 수 있는 요소가 되고 있음에 주목하고 있다. 특히 본연구에서는 APA의 개념적 배경과 실질적인 전달 단계 즉 노인의 상태 평가와 개별화된 지원계획 수립에 참여하는 사회의료팀의 동반자적 역할 및 평가 도구(AGGIR)활용과 지원 계획(Plan d’aide)의 고안 방식을 면밀히 고찰하고 있다. 한국의 노인장기요양보험 도입은 보편주의적 사회서비스 체계 마련, 노인 돌봄의사회화라는 새로운 복지 패러다임으로의 전환을 가져왔다고 평가된다. 그러나 복지사각지대, 서비스의 질, 시장화와 관련된 문제점들이 부각되면서 노인 부양 정책 시행에 있어서의 공공성 확보를 위한 제도적 지원과 정부의 역할 수행에 대한 제고의필요성이 강조되고 있는 상황이다. 이러한 점에서 공공성을 기반으로 하는 사회복지제도를 고수하며 복지 국가의 위기에 대응해온 프랑스의 노인 부양 정책은 주목할만하다고 본다. 이에 본 연구의 목적은 기존의 사회복지 관련 연구에서 중점 논의되지 않았던 프랑스의 장기요양제도에 해당하는 개인별자립성수당(APA : AllocationPersonnalise d’Autonomie)1)의 제도적 성격과 전달 단계에서의 특징들이 우리나라의 장기요양제도에 주는 시사점을 도출하는데 있다.
Abstract
In this study, the evolution of elderly care policy and the background of long-term caresystem’s adoption in France is researched in-depth. How the concept of this APA isinterpreted and applied in its implementation as well as researched attentively in terms of political and social context is also searched. This work deals with the transforming of the APA’s financial and operational authority to local government in order to expand the flexibility of policy and accessibility of service. Not only the APA’s practical deliveryphase, the assessment of the state of the elderly, examined in this analysis, but also the role of socio-medical teams who develop personalized care plans from the application of evaluation tools. Introduction of Long-term care insurance for the elderly in Korea represents a transition to a new welfare paradigm which has brought a universal social service system and the socialization of elderly care. However, due to issues with elderly care in politics such as blind spots in welfare, quality of service, and commercialization, ensuring publicness by institutional supports and the role of the government is needed. In this regard France’s elderly care policy experience which has tried to conserve the publicness to deal with the crisis of a social welfare state is noteworthy. The purpose of this study is to look for suggestions for Korean long term care insurance by institutional characteristics and the features of the delivery system ‘Personalizedautonomy allowance (APA: Allocation Personnalisée d'Autonomie)’. Actually, France’sAPA corresponding to the long-term care system for the elderly has not been discussed as a research priority in existing social welfare-related studies in Korea.
