Methods: Through a literature review of national and international academic journal articles, research reports, and policy press releases, the policy outline of community-based integrated health promotion projects in Japan is reviewed, their policy direction is systematically examined, and actual cases in local governments are investigated. Results: Japan has been implementing the “National Health Promotion Plan” and Health Japan 21. The aim of the 3rd Health Japan 21 is to continuously carry out the projects under Health Japan 21 from 2024 to 2035, with a vision to (a) realize a sustainable society wherein all citizens can live healthy and rich lives and (b) promote health creation without excluding anyone. Conclusion: Each local government must identify cases of community-based integrated health projects, undertake continual evaluations and obtain feedback on such projects, and share these experiences with other local governments.
