초록
The authors describe a patient with recurrent right side weakness during the attack of orthostatic hypotension. Neither acute nor chronic cerebral infarction was founded on brain MRI. On cerebral MR angiography, no evidence of symptomatic arterial occlusive disease was noted. Acetazolamide-SPECT revealed symmetric collateral reserves in both hemispheres. Transcranial Doppler sonography showed decline of blood flow velocities in all intracranial vessels during orthostasis but increase in the right anterior cerebral artery. These findings suggest an asymmetric failure of autoregulation was the cause of these stereotyped TIA. The patient was managed with nimodipine and barnidipine. Korean Jourmal of Stroke 2000;2(2): 225~227
