간성뇌증을 동반한 A형간염 바이러스와 E형간염 바이러스의 동시감염 1예

A Case of Coinfection of Hepatitis A and E Virus with Hepatic Encephalopathy

지난 20년 동안 우리나라의 급성 바이러스간염은 급격한 역학변화를 보여 현재 국내에서는 A형간염이 급성 바이러스간염의 절반 이상을 차지하는 가장 흔한 원인이고, A형간염 환자의 80% 이상이 20대와 30대였다[2,3]. E형간염은 다른 바이러스간염에 비해 현저하게 낮은 빈도를 보이지만 산발적으로 지속적인 발생을 보이고 있다. 국내에서 HAV와 HEV의 동시감염에 대한 보고는 드문 상태이며 그 임상경과에 대한 정보가 거의 없는 실정이지만, 최근 4개 종합병원에서 전향적으로 시행한 급성 바이러스간염 연구의 중간결과에 따르면 총 245명의 급성 간염 환자들 중 19명(7.8%)의 환자에서 A형간염과 E형간염의 동시감염이 확인되었다[3]. 이 연구결과로 우리나라에서도 A형간염과 E형간염의 동시감염이 드물지 않게 발생하고 있음을 확인할 수 있고, 향후 이에 대한 많은 연구가 필요할 것이다. 본 증례는 A형간염으로 진단되어 보존적 치료를 하던 중에 급격한 상태 악화를 보였으며 이후 E형간염의 동시감염을 확인한 경우로서, 급성 A형간염에서 간성뇌증과 같은 비전형적인 임상경과를 보이는 경우에는 동일한 전염 경로를 가진 E형간염의 동시감염 여부도 확인해야 할 것으로 생각된다.

Abstract

With recent hygienic improvements in Korea, the occurrence of overt acute viral hepatitis A in adults is increasing. Acute viral hepatitis E, which has seldom been reported, has also been increasing in Korea over the past few years. Reports regarding coinfection of hepatitis A virus (HAV) and hepatitis E virus (HEV) and their clinical courses are very rare. Acute hepatitis A is usually improved by conservative management, but coinfection of HAV and HEV may lead to severe forms of disease. When a patient with acute hepatitis A presents with an atypical clinical course such as hepatic encephalopathy, HEV infection, which has identical transmission routes to HAV, must be considered. We report a case of coinfection of HAV and HEV with hepatic encephalopathy. (Korean J Med 2011;80:S101-S105)
