상피성 난소암 환자의 예후 인자 분석

초록

=목적: 본 연구는 상피성 난소암 환자에서 생존율과 무병 생존 기간에 가장 영향을 미치는 예후인자를 알아보기 위해 시행하였다.연구 방법: 1997년 1월부터 2003년 12월까지 본원에서 상피성 난소암으로 일차적으로 수술적 치료를 시행받고, 수술 후 항암화학요법을 받은 80명을 대상으로 의무기록과 수술 기록을 검토하여 연령, 수술적 병기, 조직학적 분화도, 세포 유형 및 잔류 종양의 크기를 예후 인자로 하여 비교하였다. 생존 분석은 Kaplan-Meier survival analysis를 통해 생존 곡선을 구하였고, log-rank test로 유의성을 검증하였다. 이후 독립변수들에 대해 Cox's proportional hazards model을 사용하여 다변량 분석을 시행하였다.결과: 무병 생존율에 있어서 단변량 분석에서는 연령, 조직학적 분화도, 세포의 유형은 유의한 차이를 보이지 않았다. 그러나 수술적 병기와 잔류 종양의 크기는 생존율에 의미있게 영향을 미치는 것으로 나타났으며, 이들간의 다변량 분석 결과 최종적으로 잔류 종양의 크기만이 예후에 의미있게 영향을 미치는 것으로 나타났다. 한편 생존율 분석도 이와 동일하게 수술적 병기, 잔류 종양의 크기는 단변량 분석에서 의미있는 것으로 분석되었으나, 다변량 분석에서는 잔류 종양의 크기만이 예후에 의미있는 영향을 미치는 것으로 분석되었다.
