임상적 병기 I-II 자궁내막암 환자의 병기설정 수술시 양측 난관난소절제술 유무에 따른 예후의 비교

초록

=목적: 임상적 병기 I기 와 II기인 환자를 대상으로 수술 시 양측 난소난관절제술을 시행 받은 군과 시행 받지 않은 군을 비교하여 전체 생존율과 무병 생존율, 및 수술 병리학적인 소견을 비교하여 초기 자궁내막암 환자에서 병기설정 수술시 양측 난관난소절제술의 타당성 여부를 확인 하고자 하였다. 연구방법: 1994년 1월 1일부터 2004년 12월 31일 까지 자궁내막암으로 병기설정 수술을 받은 임상적 병기 I기 와 II기인 178명의 환자들의 챠트를 후향적으로 조사하였으며, 통계청 자료를 통해 환자의 생존 여부를 확인하였다. 수술 시 양측 난관난소절제술을 시행한 군과 시행하지 않은 군간의 전체 생존율, 무병생존율, 및 수술병리학적인 결과를 비교하였다.결과: 임상적 병기 I기인 환자는 160명, 병기 II기인 환자는 18명이었으며, 이중 수술 시 양측 난소난관절제술을 시행 받은 환자는 142명, 시행 받지 않은 환자는 36명이었다. 수술 후 병리 조직 소견은 대부분 자궁내막양선암 (93.8%), 분화도 I (56.1%)이었다. 양측 난관난소절제술을 시행 받지 않은 환자가 시행 받은 환자에 비해 더 젊고 (평균 나이 40.75세 vs 55.8세), 자궁근층 침범이 덜 한 점 외에 두 군 간의 수술-병리학적인 소견 차이는 없었다. 평균 29개월 간의 추적 기간 동안 두 군간의 전체 생존율과 무병 생존율의 차이는 없었다. Cox regression으로 다변량 분석한 결과 병기 외에는 전체 생존율 및 무병 생존율에 의미있게 영향을 미치는 요인은 없었다. 골반 및 대동맥 임파전이도 두 군간의 차이는 없었다.
