초록

태아 치료가 새로운 개척분야로 등장하면서, 2002년 이전에는 주로 성공적인 증례 보고 혹은 증례 모음 등에 지나지 않았다. 그러나 새로운 치료법이 등장하여 정립되려면 기존의 표준 처치법과의 비교된 다기관 무작위 대조시험이 필요하다.58 이런 근거 중심 (evidence- based)의 임상 경향이 2002년이 지나면서 뚜렷해져, 각 질환의 경우에서 태아 치료법들이 정리, 확립되기 시작했다. 대표적인 예가 선천성 횡경막 탈장시 내시경을 이용한 기관 폐쇄라는 태아 수술이 종래의 표준처치에 비해 장점이 없었고, 쌍태아간 수혈 증후군의 치료시 양수 감소법보다 내시경을 이용한 레이저 응고법이 우수하였다. 또 다른 중요한 경향은, 장기적인 예후까지도 합병증없이 안전해야 하므로 최소 침습성, 접근성처치 및 진단 방법이 각광을 받고 있다. 대표적인 예가 태아 빈혈을 알기 위해 위험성이 큰 제대 천자보다 태아 중뇌동맥 도플러 검사 등이 새로이 개발되었고, 태아의 단일 유전자질환을 진단하기 위해 침습성이 큰 용모막 검사보다는 임신부 혈청내 세포유리 태아 DNA로 진단하는 비침습성 방법들이 대두되었다. 선천성 혈액질환 및 유전 질환시 태아 줄기세포이식이 면역 결핍성 질환에서만 효율성이 있다. 그 이외 질환에서나 유전자 치료 등은 아직 실용화되지는 않았지만 윤리적, 기술적 발전이 계속된다면 약속된 장래가 머지 않을 것이다.
