자궁근종환자에서 자궁의 무게에 따른 질식전자궁적출술의 이환률에 관한 연구

=목적: 자궁의 무게에 따른 질식자궁적출술의 합병증을 비교분석하여 질식자궁적출술의 안정성과 용이성을 알아보고자 하였다.연구 방법: 2003년 6월부터 2004년 6월까지 본원 산부인과에서 질식 전자궁적출술을 시행받은 환자 중 자궁탈출증을 제외한 549명을 대상으로 하였으며 이를 자궁무게를 180 gm 미만인 132명 (Group I)과 180 gm 이상 280 gm 미만인 238명 (Group II), 280 gm 이상 380 gm 미만인 100명 (Group III), 380 gm 이상인 79명 (Group IV)으로 나누어 비교하였다. 수술 후 의무기록을 토대로 후향적 분석을 통해 조직 검사상 자궁근종이나 자궁선근종이 나온 환자들만을 대상으로 분석하여 자궁 무게에 따른 질식자궁적출술의 이환률의 상관관계를 알아보고자 하였다.결과: 각 그룹별 나이 (45.9±4.94세), 분만력 (1.88±0.81), 수술시간 (80.72±20.86분), 입원기간 (9.08±0.54일), 동반된 수술에선 유의한 차이점이 없었으며 전반적인 합병증 발생률에서도 (수술 전 후 헤모글로빈 차이 M=1.25, 합병증 발생률 9.8%) 유의한 차이는 없었다. 그러나 수술시간 (P=0.00), 세정술 (P=0.00)의 사용은 무게가 증가할수록 유의한 차이점을 나타내었다.
