산후출혈처치에 있어 Sengstaken-Blankemore 튜브의 효용성

=목적: 고식적 치료로 조절되지 않는 산후출혈 환자에서 S-B 튜브 (Sengstaken-Blankemore tube)를 이용한 tamponade balloon 치료의 출혈조절 효과와 다른 침습적인 추가치료를 결정하기 전에 고려할 수 있는 중재적인 치료역할에 대해서 알아보고자 하였다.연구 방법: 2000년 2월부터 2005년 9월까지 대학교병원 산부인과에서 경험한 90명의 산후출혈 환자를 대상으로 하였다. 2004년도 이후 약물처치에 반응 없는 산후출혈에서 침습적인 치료의 결정 이전에 modified S-B 튜브를 적극적으로 적용하였는데, 이들 중 17명의 환자에서 출혈 조절을 위해 S-B 튜브가 적용되었다. 대상 환자들의 의무기록을 분석하여 임상적 상황, 산후출혈의 원인, ballooning 양 및 지속시간, S-B 튜브 처치 실패 시 시행된 추가치료 및 합병증 등 여러 요소를 조사하였다.결과: Tamponade balloon치료의 성공률을 보면 전체 17명 중 11명 (64.7%)의 환자에서 성공적으로 출혈이 조절되었다. 출혈의 원인에 따른 성공률을 보면 자궁이완증 환자에서 80%, 자궁퇴축부전 환자는 100%의 성공률을 보인 반면, 태반이상으로 인한 출혈에는 20%의 성공률을 나타내어 원인에 따른 성공률에 차이가 있었다. 2004년도 이후 침습적 치료의 결정 이전에 S-B 튜브를 적극적으로 적용하였는데, 이를 적용하지 않았던 2004년 이전의 산후출혈 예와 비교한 최종 치료결과는 2004년 이전에는 8예 (17.8%)에서 자궁적출술이 이루어 진 반면, 2004년 이후에 발생한 산후출혈 환자에서는 단, 1예 (2.2%)에서만 자궁적출술이 이루어져 유의한 차이를 보였다.
