유두성 장액성 자궁내막암의 임상적 고찰: 자궁내막 선암과의 비교분석

초록

=목적: 유두상 장액성 자궁내막암은 자궁내막암 중 드문 암종에 속하지만 나쁜 예후를 보인다는 면에서 임상적으로 중요하다. 본 연구의 목적은 자궁내막 선암과 비교하여 유두상 장액성 자궁내막암 환자들의 임상적 특성을 분석하고, 치료에 대한 결과 및 예후를 알아보는 것이다.연구 방법: 1989년 5월부터 2004년 12월까지 서울아산병원에서 수술로 진단된 21명의 유두상 장액성 자궁내막암 환자와 332명의 자궁내막 선암 환자를 대상으로 하였다. 각 환자들의 의무기록, 병리보고서를 통하여 임상적 특징, 조직학적 소견, 수술적 병기, 치료 양식, 예후 및 생존율에 대하여 조사하였다.결과: 21명의 유두상 장액성 자궁내막암 환자들의 진단 당시 평균 나이는 60.9세였다. 폐경 후 여성이 18명이었으며 (85.7%), 대부분의 환자가 비정상 질출혈을 나타냈다 (75%). 자궁내막암의 위험요인으로 알려진 비만, 당뇨, 고혈압, 호르몬 대체 요법의 경험 등은 대체로 보이지 않았다. 병기 1기가 10명 (47.6%), 2기가 2명 (9.5%), 3기가 5명 (23.8%), 4기가 4명 (19.1%)이었다. 병기에 관계없이 전체 3년 생존율은 자궁내막 선암에서 97.4%이었고, 유두상 장액성 자궁내막암에서 43.1%로 나타났다. 수술을 시행 받은 21명의 유두상 장액성 자궁내막암 환자 가운데, 3명의 환자가 수술만 시행하고 보조요법을 받지 않았으며, 9명의 환자가 수술 후 방사선 치료를, 6명의 환자가 수술 후 항암 화학요법을, 그리고 3명의 환자가 수술 후 항암 화학요법과 방사선 요법을 함께 시행 받았다.
