간경화 산모의 제왕절개술 이후 발생한 대량의 복수 1예

간경화 환자의 임신율은 그 빈도가 낮은 것으로 알려져 있으나 만성간질환에 대한 치료의 발달로 간경화 환자의 임신율 및 성공적인 출산 사례도 증가하고 있다. 본 사례에서 간경화를 앓고 있던 32세의 산모가 악화된 자간전증으로 인해 31주 3일에 제왕절개술을 시행 받았다. 수술 3일 후 산모는 복부팽만을 호소하였으며 복부초음파상 대량의 복수가 발견되었다. 복수천자 및 항생제와 이뇨제 투여 후 산모는 퇴원하였으며 이후 경과 관찰에서 특이사항이 없었다. 만성간질환 환자의 임신 사례가 늘고 있지만 그러한 환자들의 이환율과 사망률은 여전히 일반 인구보다 높은 편이다. 그러므로 간경화가 동반된 임신과 합병증에 대한 처치, 적절한 치료 지침에 대한 담당 의사의 지식과 환자에 대한 주의 깊은 관찰이 필요하겠다.
Abstract
Pregnancy is a rare event in patients with cirrhosis. But recently, improvements in the treatment of chronic liver disease have resulted in higher conception rates and more successful pregnancy outcomes. A 32-year-old woman with liver cirrhosis underwent cesarean section due to aggravated preeclampsia at 31 3/7 weeks of gestation. After 3 days postoperatively, she complained abdominal distension, and abdominal sonography showed massive amount of ascites. Following paracentesis and administration of diuretics, her course was uneventful. There are increasing numbers of case reports and case series of pregnancy in liver cirrhosis. Nevertheless, morbidity and mortality remains higher than that of the general pregnant population. It is therefore important for physicians caring for pregnant patients with cirrhosis to understand how to approach potential complications and tailor management regimens appropriately.
