급성 호흡곤란 증후군이 합병된 임신 중 급성 지방간: 모자 생존 1예 보고

=저자들은 임신 중 급성 지방간 1예를 경험하였기에 임신 중 급성 지방간의 진단, 임상 경과, 생화학적 검사 소견의 변화, 합병증, 모성 및 태아의 결과를 보고하고자 한다. 31세 경산부가 임신 35주에 신물이 올라오는 증상을 주소로 내원하여 황달이 관찰되고 혈액 검사상 간 및 신장 기능부전, 파종성 혈관내 응고병증 소견을 보이며 초음파상 지방간 소견을 보여 임신 중 급성 지방간으로 진단한 후 응급 제왕 절개술을 시행하여 모성의 복수, 태변이 착색된 양수 과다를 볼 수 있었고 2330 g의 건강한 남아를 분만하였다. 분만 후 1일, 2일, 6일째 각각 핍뇨, 급성호흡곤란 증후군, 췌장염이 발생하였다. 집중치료 후 AST/ALT, 크레아티닌 및 섬유소원, PT/PTT, 혈소판과 같은 혈액응고인자는 분만 후 7-10일, 췌장염은 분만 후 24일에 회복되었다. 급성 호흡곤란 증후군을 위한 인공호흡기 치료는 분만 후 49일까지 지속되었으며, 분만 후 88일에 퇴원을 지시하였다. 증가된 총 빌리루빈은 분만 후 7일부터 감소하기 시작하였고, 총정맥영양과 관련하여 암모니아, ALK와 r-GT, AST/ALT는 분만 후 각각 6일, 7일, 11일부터 다시 상승하여 대략 인공호흡기를 제거한 후 정상으로 회복되었다.
