학령기 뇌성마비 아동의 삶의 질에 대한 연구

본 연구의 목적은 학령기 뇌성마비아동의 일반적 특성 및 임상양상, 대근육 운동기능 분류체계(GMFCS) 및 손기능분류 시스템(MACS)에 따른 삶의 질을 알아보고자 실시하였다. 경기 및 서울의 종합병원 및 복지관에서 물리치료를 받고 있는 학령기 뇌성마비아동 57명으로 일반적 특성 및 임상양상, 대근육 운동기능 분류체계(GMFCS) 및 손기능분류 시스템(MACS)을 측정하고, 삶의 질을 알아보기 위하여 한국어판 KIDSCREEN 52-HRQOL을 사용하였다. 그 결과 일반적 특성 및 임상양상에서 분만방식(p < .05), 출생 시 체중(p < .01),마비유형(p < .05), 학업형태(p < .01), 대근육 운동기능 분류체계(GMFCS)(p < .001) 및 손기능분류 시스템(MACS)(p < .001)에 따라 삶의 질에 유의한 차이를 보였고, 대근육 운동기능분류체계(GMFCS)에 따른 삶의 질 차이는 신체활동과 건강(p < .001), 자유시간, 학교와 공부, 괴롭힘(p < .05), 전체 삶의 질(p < .01)에서 유의한 것으로 나타났으며, 손기능 분류시스템(MACS)에 따른 삶의 질 차이는 신체활동과 건강(p <.001), 친구들(p <.05), 전체 삶의 질(p < .01)에서 유의한 것으로 나타났다. 본 연구를 통하여 학령기 뇌성마비아동들의 삶의 질과관련된 인자를 파악하는데 기초자료로 제공될 것이라 생각하며, 더불어 뇌성마비아동들의 삶의 질을 높일 수 있도록 치료 프로그램을 계획하고 적용하는 것이 필요하다고 생각한다.
Abstract
The purpose of this study was to compare the quality of life in school-aged children with cerebral palsy. The subjects were 57 patients(7~12 years old)school-aged children with CP. The Gross Motor Function Classification System,(GMFCS), Manual Ability Classification System(MACS), KIDSCREEN 52-HRQOL and questionnaire were used for this study in order to measure children’s quality of life in school-aged children with cerebral palsy. As a result, In general characteristics, it showed meaning difference in childbirth(delivery) style(p < .05),birthweight(p < .01), paralysis type(p < .05), studies form(p < .01), GMFCS(p < .001),MACS(p < .001) classification in quality of life by general item of school-aged cerebral palsy child(p < .001). Difference of quality of life in school-aged children with cerebral palsy in according with GMFCS and MACS calssification were statistically significant in the quality of life. the results of the school-aged cerebral palsy children quality of life and several common factors, depends on the ability of children(GMFCS, MACS difference). These result suggest that provide basic data they can make practical treatment goal and intervention plane and will do much contributing the rehabilitation of cerebral palsy.
