ADHD 아동의 애착과 문제행동을 위한 치료놀이를 활용한 미술치료 단일사례연구

본 연구 목적은 치료놀이를 활용한 미술치료가 ADHD 아동의 애착과 문제행동에 미치는 영향을 알아보는데 있다. 연구 대상은 소아정신과에서 ADHD 진단을 받은 초등학교 1학년 남자아동으로 부모가 함께 참여하였으며, 주 1회, 60~90분간, 총 25회기 치료놀이를 활용한 미술치료를 진행하였다. 측정 도구는 구조화된 부모-아동 상호작용 평가, 동적가족화(KFD), 아동ㆍ청소년 행동평가척도(K-CBCL), 학교생활화(KSD)를 사용하였다. 본 연구의 결과는 다음과 같다. 첫째, 구조화된 부모-아동 상호작용 평가에서 점수가 증가한 것으로 보아 ADHD 아동의 애착에 긍정적인 영향을 미쳤으며, 동적가족화(KFD)에서 가족간의 정서적 상호작용에 대한 모습을 보이고 수치가 증가하여, ADHD 아동의 애착에 긍정적인 영향을 미쳤다. 둘째, 아동ㆍ청소년 행동평가척도(K-CBCL)의 공격성 척도, 주의집중문제 척도에서 모두 70점 이하의 정상범위에 해당하여 ADHD 아동의 문제행동에 긍정적인 영향을 미쳤으며, 학교생활화(KSD)에서 ADHD 아동의 문제행동에 긍정적인 영향을 미쳤다. 셋째, 단계별 활동내용 분석에서도 양육과 돌봄, 감정의 표현과 수용, 지지와 격려를 통해 안정애착을 경험함으로써 문제행동이 완화된 모습을 보였다.
Abstract
The purpose of this study is to examine how Theraplay-based art therapy affects attachment and behavioral problems in a child with attention deficit hyperactivity disorder(ADHD). The study was conducted on parents and their first-grade elementary school boy who had been diagnosed with ADHD by a child psychiatrist, and for 25 weeks, the Theraplay-based art therapy program had been implemented 60~90minutes a week. Structured parent-child interaction, Kinetic Family Drawing(KFD), Korea-Child Behavior Checklist(K-CBCL) and Kinetic School Drawing(KSD) tests has been used in this study to examine the effects. The results of this study can be summarized as follows: First, the structured parent-child interaction assessments score has been improved, suggesting a positive effect on the ADHD child’s attachment relationship and in the KFD test the test score has been improved and the family showed emotional interaction. It shows that the Theraplay-based art therapy also helped improve the ADHD child’s attachment relationship. Second, in K-CBCL on the scale of Aggressive Behavior, Attention Problems, the T scores of both parents were found in the normal range under 70. Theraplay-based art therapy was found to bring a positive change to behavior problems in the ADHD child, Judging from that the child depicted his friends that shows the improvement of a friendship in Kinetic School Drawing(KSD) tests, Theraplay-based art therapy improved the ADHD child’s attach ment relationship. Third, the examination of changes at each stage of the Theraplay- based art therapy program showed that it seems that the art therapy intervention helped the child experience a sound attachment relationship based on parenting & caring, emotional expression & acceptance and support & encouragement, thereby reducing his behavioral problems.
