Abstract

Background: Extended-spectrum β-lactamase (ESBL)-producing Klebsiella pneumoniae can form biofilms; however, whether this biofilm forma- tion is associated with antibiotic resistance profiles and the presence of virulence genes remains unclear. This study has aimed to evaluate wheth- er biofilm formation was associated with antibiotic susceptibility patterns and the prevalence of virulence genes in ESBL-producing K. pneumoniae. Methods: A total of 202 K. pneumoniae , including 142 ESBL-producing isolates, were collected, and their biofilm-forming ability was determined using the tissue culture plate method. The antimicrobial susceptibility patterns of the ESBL-producing isolates were reviewed, and the presence of virulence genes (fimH, mrkD, and irp2 ) was detected by polymerase chain reaction (PCR). Results: ESBL-producing isolates showed higher rates of biofilm formation (94.4% vs. 81.7%, P = 0.005) and a greater proportion of moderate/ strong biofilm formation (76.8% vs. 60.0%, P = 0.016), than non-ESBL-producing isolates. The incidences of overall biofilm formation and moder- ate/strong biofilm formation did not differ between specimen types. Cefoxitin and tigecycline susceptibilities were significantly lower among ESBL- producing isolates with moderate/strong biofilm formation. The prevalence of virulence genes did not differ between non/weak and moderate/ strong biofilm-forming isolates. Conclusions: A large proportion of ESBL-producing K. pneumoniae strains form biofilms. The specimen type was not related to the intensity of biofilm formation. Moderate or strong biofilm-forming strains may be associated with decreased susceptibility to cefoxitin and tigecycline, where- as the presence of virulence genes does not appear to influence biofilm-forming capacity.
