국내 일부 3차 의료기관에서 실시하는 종합검진의 검사항목에 관한 근거 조사

연구배경: 최근 각종 암과 만성 질환에 대한 관심이 증가하면서 각급 병․의원에서 많은 종합검진 프로그램이 시행되고 있다. 그러나 대부분 개개인의 특성(나이, 성별, 직업)과 위험요인을 고려하지 않은 검사 위주의 검진프로그램으로 각 검사의 의학적 근거에 대한 논란이 끊이지 않고 있다. 이에 국내 병원의 검진프로그램 내용을 알아보고, 시행하는 검사 항목에 대한 의학적 근거를 평가하여 올바른 검진프로그램을 시행하는 데 도움이 되고자 본 연구를 시행하였다. 방법: 국내 6개 병원과 건강보험공단의 검진프로그램을 대상으로 각 병원 검진센터와 건강보험공단의 인터넷 홈페이지를 통해 검사항목을 수집하였다. 중점 대상 질환을 정한 뒤 수집된 프로그램의 검사항목을 질환별로 분류하였다. 이를 여러 단체(USPSTF, CTFPHC, 한국인의 평생건강관리 등)의 권고안과 비교, 분석해 의학적 근거를 평가하였다. 결과: 여러 단체가 검진항목에서 제외하기를 권고하는 각종 종양 표지자와 복부 초음파, VDRL, C형 간염 항체 검사가 6개 병원의 검진프로그램에 일률적으로 포함되어 있었다. 그리고 아직 선별검사로서 근거가 불충분한 많은 검사들이 시행되고 있었다. 개인별 특성이나 위험도와는 상관없이 수검자의 기호나 경제적 능력에 따라 검진 프로그램과 주기를 선택할 수 있는 문제점도 있었다. 결론: 본 연구에서 조사한 국내 병원의 검진 프로그램은 선별 검사로서의 그 유용성이 확립되지 않은 검사들을 다수 검진 항목에 포함시키고 있었다. 따라서 수검자 개개인의 특성과 위험요인을 고려한 맞춤형 검진이 이뤄져야 한다.
