= Abstract = Objectives: High nursing turnover rates pose significant challenges to healthcare institutions' operational stability and patient safety. Since 2022, the Ministry of Health and Welfare has been implementing a shift improvement pilot project to reduce turnover rates and enhance quality of life through improved working conditions. This study evaluated the effectiveness of the shift improvement pilot project by comparing job satisfaction and turnover intention between participating and non-participating nurses. Methods: A cross-sectional study was conducted among 354 nurses (162 participants, 192 non-participants) from five national Veterans Hospitals (general hospitals) across South Korea from January to February 2025. Data were collected using structured online questionnaires. Statistical analyses included descriptive statistics, chi-square tests, independent t-tests, Spearman correlation analysis, and multivariable logistic regression. Results: Nurses in improved shift systems showed significantly better job characteristics, particularly in ensuring 2-day rest periods after night shifts (95.1% vs. 19.3%, p<0.001). Job satisfaction was significantly higher (122.7 vs. 108.7, p<0.001), while turnover intention was lower (36.5 vs. 40.8, p<0.001). Logistic regression showed improved shifts were a strong protective factor against low job satisfaction (OR=0.246) and high turnover intention (OR=0.269). Lack of guaranteed 2-day rest after night shifts increased the risk of low job satisfaction (3.8-fold) and high turnover intention (2.5-fold). Conclusions: Nurses in the shift improvement pilot project reported significantly better working conditions, higher job satisfaction, and lower turnover intention compared to those in general wards. The findings demonstrate that shift improvements are associated with turnover intention through enhanced working conditions and improved job satisfaction, indicating the need for comprehensive working environment improvements in nursing workforce policy.
