고난의 행군 시기 전후의 북한 의료인과 주민들의 보건의료 경험 분석과 시사점

초록

본 연구에서는 세계보건기구(WHO)에서 제시한 ‘보건의료체계 여섯 가지 구성요소’ 분석체 계를 활용하여 고난의 행군 시기 전후 북한의 보건의료서비스를 실제로 이용하였던 의료인 출신과 일반인 출신 북한이탈주민을 심층 면담한다. 이를 통해 북한 주민이 가진 건강에 대한 의식과 행동을 종합적으로 파악하고 현재의 북한보건의료 상황을 유추해본다. 분석적 내용을 바탕으로 향후 효율적인 남북보건의료 협력 활성화에 고려할 사항은 다음 과 같다. 첫째, 북한 주민들의 낮은 건강의식 개선이 가능하도록 일차보건의료 영역에 초점을 맞춘 교류 지원이 필요하다. 둘째, 예방의학 차원에의 건강증진 교육프로그램 지 원 및 의료인 교육을 바탕으로 인적 교류를 도모한다. 셋째, 북한 내 건강 불평등 완화를 위해 국제사회와의 현실적 공조 방안 마련이 요구된다. 곧, 고난의 행군 시기 이후 20여 년의 세월이 흐른 지금도 여전히 열악한 보건의료적 상황에서 생활하고 있는 북한 주민 의 삶과 건강이 조금이나마 향상되기 위한 현실적 노력이 절실히 요구되는 때이다.

Abstract

This study conducts the Focus Group Interview on North Korean defectors (healthcare workers and residents) who have experienced healthcare service while in North Korea using the WHO’s health system building blocks. Through this, we will comprehensively understand the health awareness and behavior of North Koreans and infer the current healthcare conditions of North Korea. Based on the analysis, the followings are the considerations for promoting effective inter-Korean health and medical cooperation in the future. First, it is necessary to fully exchange supports in the primary health care sector so that North Koreans can improve their low health awareness. Second, exchange of healthcare human resources is needed to promote with health promotion education and training of healthcare workers in terms of preventive medicine. Third, cooperation agenda with the international community is required to mitigate health inequality in North Korea. Thus, it is a time to have practical efforts for improving the lives and health status of North Koreans, who are still living in poor healthcare conditions over more than two decades since the period of the Arduous March.
