알코올의존 노숙인 치료공동체 프로그램의 효과성에 관한 연구: 3개월 입소유지자의 심리사회적 변화를 중심으로

초록

본 연구의 목적은 알코올의존 치료공동체(therapeutic community) 프로그램에 참여하는 노숙인의 회복에 필요한 다양한 심리사회적 변화를 3개월 입소기간을 중심으로 실증적으로 검증하는 것이다. 본 연구는 선행연구 고찰을 근거로 알코올의존 치료공동체 프로그램을 운영하는 한 노숙인 전문재활쉼터의 입소자 16명의 입소 직후와 3개월 시점에 측정한 자존감, 금주 자기효능감, 변화단계, 치료공동체 회복과정을 대응표본 T-test로 비교분석하였다. 분석 결과는 다음과 같다. 첫째, 클라이언트의 자존감과 금주 자기효능감 모두 통계적으로 유의미한 수준에서 긍정적인 변화가 있는 것으로 나타났다. 둘째, 변화단계의 하위 영역(인식, 양가성, 실천)의 변화는 나타나지 않았다. 셋째, 회복과정에 대한 분석에서 CAI와 SAS의 일부 영역과 전체 합산값의 변화가 통계적으로 유의미하게 나타났다. 이러한 결과로 치료공동체 프로그램을 통하여 클라이언트의 긍정적인 심리사회적 변화가 제한된 수준에서 나타나는 것으로 확인되었다. 본 연구 결과를 바탕으로 치료공동체 프로그램의 효과성에 대한 연구와 실천을 위한 이론적, 실천적 함의와 제한점에 대해 논의하였다.

Abstract

The purpose of the study is to examine empirically psycho-social changes of homeless participants in a therapeutic community(TC) of alcohol dependence program for three months. According to previous studies, it is significantly important to analyze the particular changes under the treatment in order to expect the effect of the program- abstinence of alcohol and rehabilitation. It is also recognized that the first three months are critical points to the effect of alcohol dependency treatment. To identify the effects of the TC program, 16 homeless persons who joined a therapeutic community for three months were measured by scales concerning self-esteem, abstinence self-efficacy, the stages of change and recovery process(CAI, CAS, SAS). And paired groups t-test was used. The results are as follows. First, the clients' self-esteem, abstention, self-efficacy were statistically significant and showed positive change. Second, the stage of the change process(recognition, ambivalence, taking steps) did not appear in the analysis. Finally, the changes of part of CAI and SAS and total score were statistically significant in the process of rehabilitation. Through these results, therapeutic community program is somewhat effective to handle the participants' psycho-social change positively. The study implies the effect of therapeutic community program and suggests practice implication and limitation.
