장기요양보험의 급여충분성 탐색: 장기요양서비스 미이용자 분석을 중심으로

본 연구는 향후 장기요양서비스가 대상자의 욕구 및 상태에 맞춰 충분히 제공될 수 있도록 제도적 변화가 필요하다는 문제의식에 기반해 현재의 장기요양보험제도의 급여(서비스)가 충분한가를 진단해 보고 도출되는 한계점을 개선하기 위한 검토과제를 제시하였다. 이를 실증적으로 살펴보기 위해, 장기요양 미이용자의 특성과 미이용 사유를 중점적으로 분석하였는데 미이용자의 사유에서는 1)요양병원이용(의료적 처치), 2)요양병원이용(의료적 처치 불필요, 요양목적 등), 3)가족 등에 의한 직접요양의 순으로 빈도가 높게 나타났으며 각 사유별 미이용자의 성별, 연령, 장기요양등급, 자격기준이 통계적으로 유의한 차이가 있는 것으로 나타났다. 이렇게 구체적으로 살펴본 결과, 지난 12년간 노인장기요양보험제도가 제도적 성숙을 이뤄왔지만 여전히 의료서비스의 적절한 제공이 미흡하고, 현재의 요양서비스만으로 재가생활이 어려워 결국에는 요양병원 이용이라는 선택으로 이어질 수 있을 것이라는 검증이 가능했으며 이에 대한 적절한 정책적 보완이 이뤄져야 할 것으로 보인다.
Abstract
In this study, the current status of the blind spots of the long-term care insurance system was reviewed in general in order to promote institutional changes so that long-term care services can be provided in accordance with the needs and conditions of the recipients. The following unused reasons 1) the use of a nursing hospital (medical treatment), 2) the use of a nursing hospital (not requiring medical treatment) and 3) direct care by the family showed that the gender, age, grade, and qualification criteria were statistically significant. As a result, the long-term care insurance system has been achieving institutional maturity for the past 12 years, but adequate provision of medical services is still insufficient. In addition, it was found that there were cases where the family was still burdened because they were not satisfied with the service.
