유방암 환자의 호르몬치료에 대한 지식과 약물복용이행에 관한 연구

목적: 본 연구는 유방암 환자의 호르몬치료에 대한 지식과 약물복용이행을 파악하고 약물복용이행에 영향하는 요인을 파악하고자 시행된 조사 연구이다. 방법: 자료 수집은 암 센터가 있는 3개의 종합병원에서 유방암 진단 후 수술과 항암화학요법 치료를 마치고 보조적 호르몬치료 요법을 시행 중이고, 호르몬치료를 시작한지 3개월 이상 지난 136명을 대상으로 2014년 7월 1일부터 8월 14일까지 이루어 졌다. 수집된 자료는 Independent T-test, ANOVA, Scheffe test, and multiple regression 분석하였다. 결과: 호르몬치료에 대한 지식은 9점 만점에 평균 5.15(±2.22)이며, 약물복용이행은 8점 만점에 평균 5.76(±1.65) 점으로 나타났다. 대상자의 특성에 따른 약물복용이행의 차이 분석결과 나이가 적을수록, 호르몬치료 기간이 짧을수록, 호르몬치료 결정에 적극적으로 참여 할수록, 일상생활에 긍정적 영향을 미칠수록, 완치신념이 높을수록 약물복용 이행이 높게 나타났다. 호르몬치료 지식과 약물복용이행 간에는 약한 상관관계가 있었으며, 약물복용 이행에 영향을 미치는 요인은 나이, 호르몬치료 기간, 호르몬치료가 일상생활에 미치는 영향, 완치신념으로 나타났고 설명력은 25.2%였다. 결론: 약물복용이행을 높이기 위해서 부작용과 치료목적에 대한 교육을 제공하고 호르몬치료 결정시 환자를 적극 참여시키는 것이 필요하고 나이가 많거나 호르몬치료기간이 긴 환자들에게 반복교육을 통한 간호중재를 제공한다면 약물복용이 증가할 수 있을 것이다. 더불어 약물복용을 잊어버리지 않고 기억할 수 있도록 돕는 간호중재 개발이 필요하다.
Abstract
Purpose: The purpose of this study was to investigate the levels of knowledge and medication adherence to hormonal therapy (HT) and to identify the factors influencing medication adherence for patients with breast cancer. Methods: Data were collected from 136 patients undergoing adjuvant HT for breast cancer in 3 general hospitals from July 1 to August 14 in 2014 using self-report questionnaires. Data were analyzed using descriptive statistics, independent t-test, ANOVA, Scheffè test, and multiple regression. Results: The average of knowledge about HT was 5.15±2.22 (Max 9), and that of medication adherence was 5.76±1.65 (Max 8). Younger age, shorter duration of HT, more active participation in decision making for treatment, positive perception for impacts of HT, and stronger belief in cure were influencing factors on higher adherence level. Age, duration of HT, and perception on the impacts of hormonal therapy, and belief in cure explained 25.2% of the adherence. Conclusion: To improve the treatment adherence to hormonal therapy, patient education and involvement in decision making, and the tailored intervention for the patients with older age, and long treatment period of HT are needed. Additionally, the strategies for diminishing unintentional forgetting is necessary to be developed
