항암화학요법에 관한 개별교육이 유방암환자의 지식과 자가 간호수행에 미치는 효과

초록

본 연구는 유방암진단을 받고 유방전절제술이나 부분절제수술 후 예방 목적 또는 치료 목적으로 항암화학요법을 받기 시작한 환자에게 제공된 개별 환자교육이 자가간호 지식, 자가간호 수행에 미치는 효과를 파악하기 위하여 시도하였다. 연구 설계는 유사 실험 연구로 비동등성 대조군 전후 시차 설계로 연구기간은 2010년 1월1일부터 2010년 6월 30일까지이었으며, 연구대상은 항암요법을 받기 위해 입원한 유방암환자로, 2010년 1월 1일부터 2010년 6월30일까지 대조군과 실험군을 각 29명씩 선정하였다. 실험군과 대조군 모두에게 항암전문간호사가 항암교육을 실시하였다. 개별교육 자료는 항암화학요법의 정의, 항암화학요법의 부작용과 대처방안, 식이와 영양 및 일상생활로 구성되었다. 수집된 자료는 SPSS Windows 16.0 프로그램을 사용하여 연구목적에 따라 통계 처리하였으며, 백분율, 평균, 표준편차, χ²-test, t-test, Pearson Correlation로 분석하였다. 연구결과는 다음과 같다. 1) 항암화학요법에 관한 개별교육을 받은 실험군은 개별교육을 받지 않은 대조군보다 자가간호 지식 정도가 유의하게 증가되었다(t=-3.620, p=.001). 또한 항암화학요법에 관한 개별교육 적용 전과 후 자가간호 지식 정도의 변화를 검증한 결과 대조군(t=-7.01, p=.489)은 통계적으로 유의한 차이가 없었으나 실험군 (t=-5.147, p=.001)은 통계적으로 유의한 차이를 나타내 항암화학요법에 대한 개별교육이 유방암환자의 자가간호 지식을 증가 시키는 것으로 나타났다. 2) 항암화학요법에 관한 개별교육을 받은 실험군은 개별교육을 받지 않은 대조군보다 자가간호 수행 정도가 증가되었다(t=-2.994, p=.004). 또한 항암화학요법에 관한 개별교육 적용 전과 후 실험군, 대조군의 자가간호 수행정도의 변화를 보면 대조군(t=-1.128 p=.269)의 경우 통계적으로 유의한 차이가 없었으나 실험군(t=-6.543, p=.000)의 경우 통계적으로 유의하게 증가된 것으로 나타났다. 즉 항암화학요법에 대한 개별교육 후 유방암 환자의 자가간호 수행정도가 증가된 것을 알 수 있다. 3) 항암화학요법에 관한 개별교육을 받는 실험군은 항암화학요법에 관한 자가간호 지식 정도와 자가간호 수행 정도 간에는 정의 상관관계가 있는 것으로 나타났다(r=.494, p=.006). 대조군에서는 자가 간호지식 정도와 자가 간호수행 정도 간에는 유의한 상관관계를 나타내지 않았다(r=-.183, p=.342). 즉 항암화학요법에 관한 자가간호 지식이 높을수록 자가간호 수행정도 또한 높다고 말할 수 있다.

Abstract

Purpose: The purpose of this study was to examine the effect of an individual education program on breast cancer patients who received chemotherapy. Methods: The quasi-experimental design was employed with a nonequivalent control group pre-post test. The study was performed from January 1 to July 30, 2010 for the patients who were firstly treated by A·C (Adriamycin, Cyclophosphamide) chemotherapy in a university hospital. Control group was recruited first to prevent treatment diffusion. The experimental group was recruited and received an individual education during 30 minutes. The collected data were analyzed using SPSS 16.0 version. χ²-test, t-test, and the Pearson correlation were implemented in order to examine the effect of the individual education among the patients who are treated by chemotherapy. Results: The experimental group had a greater improvement in self-care knowledge (t=-5.236, p=.001), and self-care performance (t=-6.543, p=.004) than did control group. Moreover, the experimental group had significantly positive correlation between self-care knowledge and self-care performance (r=.494, p=.006) as compared to the control group. Conclusion: The individual education program on cancer patients who were treated by chemotherapy could be utilized as an effective nursing intervention.
