입원환자의 상급병실 이용행태 및 상급병실 이용에 영향을 미치는 요인

우리나라는 2005년 이후 건강보험 보장성 강화를 위해 꾸준히 노력한 결과 암 등 고액․중증질환자에 대한 보장률은 상당한 성과를 보여 왔으나, 급증하는 비급여로 인해 보장률이 정체하고 있는 실정이다. 비급여 중 상당부분을 차지하고 있는 상급병실료에 대한 정책개선의 요구가 커지고 있으나, 이를 뒷받침하는 정책적 근거는 미비하다. 이에 본 연구는 입원환자의 상급병실 이용행태를 파악하고 상급병실 이용에 영향을 미치는 요인을 파악하기 위하여 병원급 이상 의료기관 입원경험이 있는 환자들을 대상으로 입원 시 병실선택권과 병실 이용행태에 대해 전화설문조사를 시행하였다. 조사결과, 상급병실 이용 환자들은 비자발적으로 병실을 선택하는 비율이 높았으며, 상급병실에서 일반병실로 이동하는 환자들은 비자발적 상급병실 이용이 특히 높은 것으로 나타났다. 의료기관의 규모가 커질수록 비자발적 병실선택 비율이 높아졌다. 상급병실 이용에는 성별, 연령, 의료보장의 종류, 소득분위, 입원 시 질환, 의료기관의 유형이 유의한 영향을 미치는 것으로 분석되었으며, 자발적 상급병실 이용에는 연령, 소득분위, 입원 시 질환, 의료기관의 유형, 진료비 부담주체가 유의한 영향을 미치는 것으로 분석되었다. 의료기관의 규모가 커질수록 병실 선택권에 제약이 있었고, 질환별 상급병실 선택권에 차이가 있었으므로 질환 및 의료기관 규모 등에 따른 별도의 고려가 필요하다. 또한, 일반병상 부족에 따른 원하지 않는 상급병실이용에 대한 문제를 근본적으로 해결하기 위해서는 대형의료기관의 환자 쏠림현상에 대한 문제해결 이 요구될 것이다.
Abstract
In Korea has tried to enhance the guarantee of Health Insurance since 2005. As a result, the coverage for expensive, severe disease such as cancer showed quite an achievement but rapidly growing uninsured rate congests the guarantee rate. The demand for policy improvement of advanced hospital ward cost, which accounts for quite significant sum of money among the uninsured, is growing but its supporting evidence is weak. This research conducted telephone survey on hospital room choice and use pattern during hospitalization targeting the patients who have the experiences at hospital level or over in order to understand use pattern of advanced ward and find the elements effect on advanced hospital room. The survey showed that many patients in advanced ward usually chose expensive rooms involuntarily this trend stands out in the patients who moved from advanced to general ward. Involuntary ward choice rate increases as the size of medical institution gets bigger. For advanced hospital room use, it analyzed that gender, age, type of insurance, income level, disease, and types of medical organization have significant influence. As for voluntary choice for advanced ward, age, income level, disease, type of medical institute, and principal of medical expense payment influence significantly.
