본 연구는 장기요양이용이 의료이용에 미치는 영향을 살펴보고자 하였으며, 이를 급성기병원이용과 요양병원이용으로 구분하여 분석하였다. 분석자료는 국민건강보험공단이 보유한 노인장기요양보험 및 건강보험 청구자료이며, 연구대상은 65세 이상 노인이면서 2009, 2010년도 장기요양 인정 및 등급판정조사를 받은 23,269명이었다. 이들을 대상으로 Two-part model을 적용하여 분석하였으며, 요양병원이용의 경우에는 내생성을 통제하기 위해 도구변수를 이용한 IV-probit, 2SLS를 시행하였다. 연구결과, 장기요양이용에 따라 급성기병원이용은 이용확률과 비용 모두 양(+)의 영향을 미쳤으나, 요양병원이용은 이용확률과 비용 모두 음(-)의 영향이 나타났다. 따라서 장기요양이용과 요양병원이용은 대체 관계에 있다고 할 수 있으며, 장기요양비용이 1% 증가할 때 요양병원비용은 0.5% 감소하는 효과가 있었다. 본 연구는 노인의 장기요양이용과 의료이용 간의 관련성을 실증 분석하였다는 점에서 의의가 있다.
Abstract
The purpose of this study was to analyze the effect of long-term care utilization on health care utilization of the elderly. The sample in this study includes 23,269 people older than 65 who are extracted from 2009-2010 long-term care insurance and health insurance claim data. The study group was divided into two groups, namely acute care hospitals and long-term care hospitals. Two part model were used for statistical analysis and instrumental variables analysis were used for controlling endogeneity in the group of long-term care hospitals. This study evaluated the impact of long-term care costs on the probability of health care utilization experience and costs of health care utilization. The results showed that an increase in long-term care costs was associated with an increase in utilization of acute care hospitals, but it was associated with a decrease in utilization of long-term care hospitals. When 1% increase in long-term care costs was associated with 0.5% decrease in costs of long-term care hospitals. On the basis of results, this study concludes that long-term care utilization and utilization of long-term care hospitals for older people are substitutes.
