외래처방 인센티브 제도의 병원 확대 가능성 평가를 위한 의료기관 현황 및 의약품 선정 담당자의 인식도 조사

본 연구는 병원급 이상 의료기관의 종별 처방관리 시스템 현황과 제도에 대한 인식을 파악하여 외래처방 인센티브 제도의 확대 적용 가능성을 평가하기 위하여 수행되었다. 이를 위하여 구조화된 설문지를 개발하여 상급종합병원, 종합병원, 병원의 의약품 선정 담당자를 대상으로 우편설문조사를 시행하였다. 조사 결과, 처방의약품 선정방식, 의약품집 보유, 개별 의사에 대한 처방평가 및 피드백 시행 여부 등 처방의약품 관리체계가 의료기관 종별로 차이가 있었다. 저가의약품 도입 용이성 및 인센티브 도입 시 약제비 절감 가능성에 대하여 상급종합병원에서 부정적인 응답이 많았다. 의료기관 종별로 외래처방 인센티브 적용에 대한 여건과 인식도에 차이가 있었으며, 제도 도입 시 불편을 최소화하고 효과를 높이기 위하여 적절한 정보제공 등 정부와 의료기관의 지원과 협조가 필요하다.
Abstract
This study was performed to assess feasibility for expanding of prescribing incentive system through survey for prescribing management system and awareness to the system by types of medical institutions. We conducted mail survey for the persons in charge of prescribing drug management in tertiary hospitals, secondary hospitals, and hospitals by the structured questionnaire. The survey results shows the difference in selection process of prescribing medicines, presence of drug formulary, individual assessment and feedback to clinicians by types of institutions. Tertiary hospitals had higher negative response to availability of less-priced drugs and possibility of reduction in pharmaceutical expenditure after introduction of the incentive system. Supports from the government and medical institutions will be needed to minimize the healthcare provider’s inconvenience and to improve the effect of the incentive system when it is expanded.
