동일성분 동일가격 정책에 대한 의사, 약사, 제약회사, 정책 전문가의 태도 : 포커스 그룹 인터뷰 결과

국내에서는 2012년 1월 약가제도 개편에 따라, 동일제제 의약품에 대해서는 동일한 상한금 액을 부여하는 원칙을 적용하게 되었고, 이에 따라 2012년 4월 1일자로 기존 등재 의약품을 대상으로 약가를 일괄 인하하였다. 본 연구에서는 동일성분 동일가격 정책에 대한 의사, 약사, 제약회사, 정책담당자의 반응을 파악하고자 포커스 그룹 인터뷰를 시행하였다. 포커스 그룹 인 터뷰는 개원의사, 병원의사, 약사, 제약회사, 보건정책 전문가인 5개 그룹으로 나누어 의약품 선택 요인, 가격 인하 정책에 대한 인식에 대해 토론을 진행하였다. 참여자들은 제도 변화에 따라 의약품의 효과는 큰 차이를 보이지 않고 가격도 더 이상 선택 에 영향을 미치지 않기 때문에 병원 봉직의사의 경우 병원에서 의약품 목록을 선정하나, 또한 의약품 선택시 제형과 부작용을 기준으로 결정한다고 답하였고, 개원의사는 제약회사 직원과 의 인간적 관계 등이 주요하게 작용한다고 답했다. 정책에 대한 인식은 부정적 경향이 컸는데, 오히려 제약회사에서 가격을 경쟁할 수 있는 기전이 차단되어, 약값이 상한선으로 수렴하는 경향을 보인다는 의견이 있었다. 또한 보험 상환에서 참조가격제와 같은 방식을 차용해 자발 적으로 가격을 인하할 수 있는 제도 도입이 필요하다는 의견이 많았다.
Abstract
Since 2012, Korea has introduced a new pricing system, which is ‘the same price to the same ingredient’ that all pharmaceuticals including off-patents and their generics were priced at same price after the expiration of patent. This study was conducted to examine attitude of prescribers, pharmacists, companies, and policy experts on the same price system to the same ingredient. In order to explore a plurality of perspectives, data were obtained from 5 focus groups composed of doctors from clinic and hospital, pharmacists, pharmaceutical companies, and policy experts. A semi-structured guide was applied in obtaining the participants’ opinions. Common themes were extracted by authors, which were used to gather results and draw conclusion. Participants acknowledged that the effectiveness of medication are equivalent, whereas the price is same. So hospital doctors select medication based on administration type and adverse drug reaction. In the other hand, doctors of clinic depend on relationship with salesman of companies. Participants concerned about the new pricing system because that policy would prohibiting spontaneous price competition. This study suggest that the promotion strategy of generic use targeting physicians and patients should be an effective intervention to lowering the price.
