청소년으로의 치석제거 보험급여 확대에 대한 비용편익 분석

연구배경 치석제거는 치주질환 예방에 효과적인 술식으로 현재 만 19세 이상의 성인에게 연 1회 전악 치석제거에 대해 급여되고 있다. 치주질환은 청소년기에 발병이 시작되고 이 시기에 질병 진행을 막는다면 성인에서 치주질환으로 이환을 막을 수 있지만 이에 대한 경제적 영향을 파악한 연구가 부족하다. 따라서 본 연구는 치석제거의 급여가 만 15세 이상의 청소년으로 확대되었을 때의 비용편익분석을 수행하였다. 연구방법 분석대상은 만 15세–만 18세 청소년이며 분석기간은 2018년 1년 동안으로 설정하였다. 편익은 치석제거 급여 확대로 인해 청소년에서 치주질환 감소로 인한 절감된 비용으로, 청소년 치주질환 환자의 치료비용과 치료를 위해 치과방문 시 소요되는 교통비용으로 추정하였다. 비용은 만 15세 이상 청소년이 급여로 인해 연 1회 치석제거를 위해 치과에 방문하였을 때 발생하는 비용으로, 스케일링 수진 비용과 이를 위해 치과방문 시 소요되는 교통비용을 포함하여 추정하였다. 연구결과 청소년으로 급여 확대 시 총 편익은 202억 5040만원, 총비용은 1151억 9324만원으로 추정되었다. 순편익은 –949 억 4284만원, 편익–비용 비는 0.176로 만 15세 이상 청소년으로 치석제거 급여확대는 비용 편익적이지 않은 것으로 평가되었다. 결론 청소년으로의 급여확대가 비용 편익적이지 않으나 영구치열이 완성되는 청소년기에 치주질환을 관리하지 않는다면 만성 치주질환으로 이환 되어 주요 발거의 원인이 될 수 있다. 청소년에서 정기적인 구강건강증진프로그램의 요구는 꾸준히 제기되어왔으며 예방 및 치료목적의 치석제거 급여 확대는 논의가 필요한 사항이다.
Abstract
Backgrounds The onset of periodontal disease commonly occurs in adolescence. Although the early management is necessary to reduce and prevent the periodontal disease, evidence on economic impact of expanding dental scaling insurance coverage to adolescents is scarce. This study aimed to evaluate the cost and benefit of expanding dental scaling insurance coverage to adolescents over the age of 15. Methods The study population were all adolescents between the ages of 15 and 18. The study period was 1 year from 1 January 2018 to 31 December 2018. The benefit was defined as saving costs for periodontal disease that were prevented with scaling, which included total treatment costs of periodontal disease and transportation costs. The cost was defined as scaling costs for all population aged 15-18, which included total medical costs of scaling for all those aged 15-18 and transportation costs. Results Estimated benefit was KRW 20,250,399,788 and the estimated cost was KRW 115,193,235,300. The benefit-to-cost ratio was 0.176 and the net benefit was KRW -94,942,835,512. Conclusions Expanding dental scaling coverage to adolescents appeared not to be cost beneficial to prevent and treat periodontal disease in Korea.
