Abstract

The purpose of this study is to analyze the impact of private health insurance on the recent surge in cataract diagnosis and surgery. Private health insurance reduces the financial burden on its subscribers, thereby promoting ex ante and ex post moral hazard. In the case of cataracts, the absence of explicit diagnostic criteria and technological advancements have led to a decrease in patients' potential risks and an improvement in their quality of life, thereby creating conditions where supplier-induced demand and subscribers' moral hazard can intersect. This study empirically analyzes the impact of private health insurance on the probability of cataract diagnosis and surgery as well as conditional costs among those who received cataract surgery. Using data from the Korean Health Panel 2019–2020, propensity score matching analysis was conducted to address selection bias of private health insurance. The findings indicate that private health insurance is associated with increased cataract diagnosis and a higher likelihood of undergoing cataract surgery, along with a 27.4% rise in surgery-related expenses. Our results suggest that the combination of supplier-induced demand and moral hazard in private health insurance may perpetuate a pattern of rapid increases in disease incidence and health care utilization.
