Abstract

This study examined income-related inequalities in unmet healthcare needs among residents of Seoul, South Korea, using the Concentration Index (CI) and the decomposition method proposed by Clarke et al. (2003). Data were drawn from the 2024 Korea Community Health Survey, and Seoul’s five functional living zones were used as spatial units of analysis to capture both between- and within-region disparities. The overall prevalence of unmet healthcare needs was 4.28%, with a CI of –0.1577, indicating a pro-poor distribution in which lower-income groups were more likely to experience access barriers. Decomposition results revealed that within-region inequality (–0.1522) accounted for a larger share of disparities than between-region inequality (-0.0077), suggesting that socio-economic stratification and overlapping income distributions across zones are key drivers. Structural factors such as financial burden, transportation difficulties, and long waiting times showed the most pronounced inequality (CI = –0.4653) despite a relatively low prevalence (0.89%), particularly in the Northwest and Central zones. Conversely, unmet needs due to personal reasons, including time constraints and mild symptoms, were more evenly distributed, and in some areas, higher-income residents reported greater unmet needs. The findings highlight the limitations of relying solely on “city average” indicators for policy design in megacities, as they mask substantial intra-urban disparities. Policy implications include the need for targeted interventions focusing on vulnerable groups within each living zone, removal of structural barriers, and integration of health, transport, and information services at the functional area level.
