Abstract
The global implementation of health taxes on unhealthy beverages and foods has expanded rapidly, with over 120 countries adopting sugar-sweetened beverage (SSB) taxes as of 2023. In South Korea, while a bill to introduce an SSB levy was proposed in 2021, it was ultimately abandoned due to the expiration of the legislative term. This study aims to propose tax design strategies for effectively introducing health taxes on unhealthy beverages and foods in South Korea. This study reviewed international health tax cases based on WHO and World Bank reports and examined South Korea’s existing tobacco and alcohol taxation systems along with the 2021 proposed bill. International case analysis revealed that excise taxes with volume-based (specific) tax structures are most effective for achieving policy goals. A comprehensive tax base is essential to minimize substitution effects, and tiered tax rates based on sugar content can induce product reformulation. Additionally, appropriate tax rates achieving sufficient price increases (WHO recommends at least 20%) and earmarked revenue allocation are necessary to enhance policy acceptance. South Korea’s 2021 proposed bill showed significant limitations, with the tax base limited to sugar-sweetened beverages while excluding artificial sweetener beverages, and tax rates insufficient to achieve meaningful health impact. Based on this review, the study presents design strategies for each core element of health tax systems. Three collection methods can be considered: excise taxes, health levies, and hybrid approaches, each with its own characteristics and benefits regarding earmarked use and administrative efficiency. Tax bases can be set in various scopes depending on policy objectives and feasibility, ranging from sugar-sweetened beverages only to comprehensive coverage including artificially sweetened beverages, high-sugar/high-sodium processed foods, and high-calorie processed foods, while typically excluding water, milk, and 100% fruit juices. For tax structure, specific taxes with tiered rates based on sugar content are effective for beverages, while ad valorem taxes based on ex-factory prices are appropriate for processed foods. Tax rates can be set at 10%, 15%, or 20% levels considering policy objectives and social acceptance. For earmarked revenue use, either health promotion funds or special health promotion accounts can be utilized depending on the collection method. Each design element of health tax is interconnected in determining policy effectiveness, making it crucial to maintain overall coherence and consistency in the tax design. This study is expected to make substantial contributions to future health tax introduction discussions in South Korea based on scientific evidence.
