술 전 눈물관 관류검사와 코눈물주머니 조영술 소견에 따른 실리콘관 삽입술의 성공률의 비교

초록

목적: 성인 눈물흘림 환자에서 술 전 코눈물관 관류검사 및 눈물주머니조영술의 소견에 따른 코눈물관 실리콘관 삽입술의 수술 성공률을 비교하고자 한다. 대상과 방법: 본원에서 코눈물관 실리콘관 삽입술을 시행 받은 142명을 대상으로 후향적으로 분석하였다. 수술 결과는 증상 및 술후 코눈물관 관류검사 결과에 따라 호전, 양호, 실패로 분류하였으며 호전과 양호를 보인 경우를 임상적 성공으로 정의하였다. 술전 코눈물관 관류검사 및 눈물주머니조영술 소견에 따른 임상적 수술 성공률을 비교 분석하였다. 결과: 술 전 코눈물관 관류검사에 따른 수술성공률은 완전관류의 경우 88.4%, 부분관류는 87.4%로 두 군 간의 성공률은 유의한 차이가 없었다(p=0.838, chi‐square test). 코눈물관 관류검사에 따른 조영술 소견 및 수술 성공률을 비교해보면, 부분관류가 되었던 환자들 중 눈물소관협착이 관찰된 경우는 66.7%, 코눈물관 협착 있는 경우에서 83.3%, 전반적인 협착이 있었던 경우에서 94.5%의 성공률을 보였다(p=0.018, chi‐square test). 결론: 술 전 코눈물관 관류검사에 따른 실리콘관 삽입술 성공률의 차이는 없었지만, 눈물주머니조영술 소견과 같이 고려했을 경우,부분관류를 보였던 환자 중에서 눈물소관 협착이 있었던 경우의 수술 성공률이 유의한 차이가 있었다. 술 전 코눈물관 관류검사 및눈물주머니조영술을 동시에 고려할 경우에는 수술 성공여부에 도움을 줄 수 있을 것이라 생각한다.

Abstract

Purpose: To compare the surgical success rate of silicone tube intubation according to the preoperative lacrimal syringing testand dacryocystography (DCG) in adult patients with epiphora. Methods: Medical records of 142 patients who underwent silicone tube intubation were retrospectively reviewed. The surgical resultswere divided into 3 groups based on postoperative symptoms and the syringing test results: Good, Fair, and Fail. Good andFair were classified into the surgical success groups. We compared the surgical success rate according to the preoperative syringingtest and stenosis site observed on dacryocystography. Results: The success rates of silicone tube intubation were 88.4% and 87.4% in eyes with preoperative syringing results of totally-passed and partially-passed syringing tests, respectively, revealing no statistically significant differences (p = 0.838, chisquaretest). However, in eyes with the preoperative syringing results of partially-passed syringing tests, the success rates were66.7% in patients with canalicular stenosis, 83.3% in patients with nasolacrimal duct stenosis, and 94.5% in those with diffusestenosis (p = 0.018, chi-square test). Conclusions: No significant differences were found in the success rates of silicone tube intubation according to preoperative syringingtest results. However, when considered together with DCG findings, the lowest success rate was found in eyes with thepartially-passed syringing test and simultaneously showed canalicular stenosis on DCG. The syringing test results consideredwith DCG findings may help predict the success rates of silicone tube intubation.
