당뇨관련 지지가 당뇨병환자의 우울에 미치는 영향

초록

당뇨병 환자들의 상당수가 우울을 경험하고 있는 것으로 나타나 이에 대한 관심이 증대되고 있다. 의료사회복지사의 주된 개입 영역 중의 하나인 당뇨병 관련 사회적 지지가 이러한 당뇨병 환자들의 우울에 영향을 미칠 것으로 생각되었다. 본 연구에서는 특히 당뇨관련 도구적, 정서적 지지가 당뇨병 환자들의 우울에 어떠한 영향을 미치는지 알아보기 위하여 서울소재 종합병원 2곳의 당뇨클리닉에 다니는 외래환자 중 편의표집한 2형 당뇨환자 151명을 대상으로 우울과 당뇨관련 지지에 관해 설문조사하였다. 연구결과 사회적 지지의 두 하위영역인 도구적 지지와 정서적지지 모두 우울과 유의미한 상관관계가 있는 것으로 나타났다. 또한 인구사회학적 특성과 질병관련 특성을 통제한 이후에도 정서적 지지는 우울을 예측하는 유의미한 변인으로 나타났다. 도구적 지지는 정서적 지지의 영향을 통제할 때 우울과 유의미한 관계가 없는 것으로 나타났다. 이와 같은 결과는 일반적인 사회적지지 외에도 당뇨병의 수용과 관련된 정서적 지지가 당뇨병 환자의 우울에 중요한 영향을 끼칠 수 있음을 밝혀주는 것이다. 기존의 당뇨교육이 주로 가족들의 도구적 지지방법에 초점을 두었던 것을 고려한다면 이후 당뇨교육에 있어서 환자에 대한 정서적지지가 정신건강이나 삶의 질에 중요한 영향을 미침을 이해하도록 하는 내용이 추가되어야 할 것으로 생각된다.

Abstract

Research on depression in people with diabetes is increasing. This study focus on the relationship between depression and diabetes-related social support, which is an important target area of intervention for social workers working with this population. Specifically, diabetes-related social support has been differentiated into 2 subscales - instrumental support and emotional support - and their relationships to depression have been examined. Subjects were drawn from 2 diabetes clinics located in Seoul, Korea, and were interviewed face to face by trained interviewers in the reception area. Findings revealed that both instrumental and emotional support were significantly correlated with depression. The relationship between depression and emotional support remained significant after controlling for the effects of demographic and illness-related characteristics, while the relationship with instrumental support was not significant. The results suggest that emotional support for diabetes may have an important impact on the depression level of people with diabetes. Considering that diabetes education for families of this population has focused on provision of instrumental support, we should rethink of the importance of emotional support for the mental health of people with diabetes and reflect this understanding into our diabetes education programs.
