노인 당뇨병환자의 우울에 영향을 미치는 요인 - 병원군과 복지관군의 비교 -

초록

최근 한국에서는 당뇨병이 급증하고 있으며, 평균수명의 연장과 의학기술의 발달로 인해 고령화 사회로 진행되어 가면서 노인 당뇨병환자의 증가도 현저해지고 있다. 당뇨병은 평생 동안 생활전반에 걸친 환자 스스로의 자기관리가 필수적인 만성질환으로 일상생활에서 상당한 심리사회적 어려움을 초래한다. 특히 노인 당뇨병환자는 당뇨병자체로 인한 어려움뿐만 아니라 노년기 발달단계 및 사회 환경적 특성으로 인해 당뇨자기관리를 저해하는 다양한 요인이 제시되고 있는데, 이중 우울은 당뇨병과의 높은 상관관계로 인해 주목받고 있다. 본 연구에서는 병원과 복지관을 이용하는 150명의 노인 당뇨병환자를 대상으로 우울에 영향을 미치는 요인을 조사하였다. 연구결과 인구학적 특성, 건강 및 질병관련 특성을 통제한 상태에서 병원군과 복지관군 모두 노인 당뇨병환자의 우울에 가장 영향을 미치는 변인은 주관적으로 인식하는 건강상태와 당뇨자기관리에 있어서 방해요인으로 나타났다. 이러한 결과는 노인 당뇨병환자에게 지식과 정보제공 중심으로 실시되고 있는 기존의 당뇨 교육적 접근이외에 다양한 건강증진프로그램의 실시 및 당뇨자기관리를 저해하는 방해요인의 해결에 필요한 원조를 제공하는 개입이 필요함을 시사한다. 본 연구의 결과는 의료사회복지사는 물론 지역사회기관에서 근무하는 사회복지사에게 당뇨병과 같은 만성질환을 가진 노인클라이언트에 대한 이해와 개입의 방향을 제시해주므로 연구의 의의를 가질 것이다.

Abstract

Recently, the elder diabetic patients are apparently increased by advancing to aging society. Diabetes causes severe psychosocial problems in daily life as a chronic disease demanded for self-care. It has reported that the depression is highly correlated with the diabetes. This study explored the factors effecting on the depression by targeting to the 150 elder diabetic patients who visited hospital and community welfare centers. Especially, focused on comparison between hospital and community diabetic patients. As a result, this study concluded that the major factors having effect on the depression are health status and barriers to diabetes self-care. This conclusion shows that it is necessary for implementing the various health promotion programs and developing the service in eliminating the barriers to diabetes self-care. Such results enhance that understanding on the elder diabetic patients to social workers working at community as well as medical social workers.
