생물심리사회이론에 기반한 자해의 위험요인 연구: 청소년 관점을 중심으로

본 연구에서는 자해 및 자살행동 경험이 있는 청소년이 지각하는 자해의 위험요인을 규명하고자 청소년을 대상으로 청소년 자해의 위험요인의 중요도를 평정하는 조사 연구를 실시하였다. 평정되는 위험요인은 본 연구에 선행된 상담자 패널 대상의 델파이 연구에서 문헌고찰 및 상담자 패널의 추천을 통해 구성되고 생물심리사회적 접근에 따라 분류된 위험요인 목록을 사용하였다. 본 연구에서는 자해 및 자살행동 경험이 있는 청소년을 대상으로 3차에 걸친 설문을 통해 각 위험요인의 중요도를 평정하고 추가적인 위험요인을 제안하도록 하였다. 연구 결과, 총 47개 위험요인이 채택되었다. 그 중 생물학적 요인으로 정신과적 어려움이 포함되었다. 심리적 요인은 기질, 부정적 자기지각, 부적응적 관계지각, 무력감, 정서적 문제, 스트레스 대처 문제, 자해/자살행동 경험과 관련한 26개 요인이 채택되었다. 그 외에 학교, 부모, 가족, 고립 및 상실 경험, 주변 반응, 부정적 관계 경험과 관련한 17개의 사회적 요인과 불안정한 거주환경 등의 환경적 요인 3개가 포함되었다. 본 연구의 제한점과 함의가 논의되었다.

Abstract

A survey was conducted to identify risk factors for self-harm as perceived by adolescents who have experienced self-harm or suicidal behavior. The list of risk factors included in the questionnaire was derived from a previous Delphi study of professional counselors and categorized using a biopsychosocial approach. Through three surveys, the adolescents rated the importance of each risk factor and suggested additional risk factors. A total of 47 risk factors were endorsed in the study. Among them, biological factors included only psychiatric problems. Twenty-six psychological factors were related to temperament, negative self-perception, maladaptive relationship perception, helplessness, emotional problems, stress coping problems, and experience of self-harm or suicidal behavior. Seventeen social factors were related to school, parents, family, isolation and loss, reactions to surroundings, and negative relationship experiences. Additionally, three environmental factors, such as an unstable living environment, were endorsed. The limitations and implications of this study are discussed.
