본 연구는 조혈모세포이식을 받은 백혈병 환자의 희망정도와 그 영향요인을 파악하여 조혈모세포이식 환자의 희망을 증진시킬 수 있는 간호중재 개발의 기초자료를 제공하기 위하여 시도하였다. 본 연구의 대상은 2001년 7월부터 2002년 6월까지 가톨릭 대학교 성모병원 조혈모세포이식 병동에서 백혈병으로 조혈모세포이식을 받은 환자 88명으로 질문지를 통하여 자료를 수집하였다. 수집된 자료는 SAS 프로그램을 이용하여 t-test, ANOVA와 Scheffé test, Pearson correlation, Stepwise multiple regression으로 분석하였다. 본 연구의 결과는 다음과 같다. 1. 대상자의 희망은 평균평점 3.39±0.35점으로 희망정도가 높았으며, 하위영역별로는 회복가능성에 대한 확신이 가장 높았고, 영적 믿음 영역이 가장 낮았다. 2. 대상자의 특성에 따른 희망은 성별, 종교 및 종교의 의미, 질병에 대한 확신정도에 따라 차이가 있었다. 3. 희망과 자아존중감은 정상관관계가 있었고, 우울, 기질불안, 상태불안은 희망과 역상관관계였다. 의사의 지지, 간호사의 지지, 삶의 만족도와는 정상관관계였다. 4. 희망에 영향을 주는 주요변인은 상태불안으로 희망을 35.4% 설명하였으며, 의사의 말이나 행동(17.1%), 기독교(11.3%), 자아존중감(7.6%)을 추가하면 희망을 71.4% 설명하였다. 결과적으로 백혈병으로 조혈모세포이식을 받고 생착이 확인된 환자의 희망정도는 높은 편이었으며, 자아존중감, 의료진의 지지, 삶의 만족도가 클수록 높았으며, 우울, 불안이 높을수록 낮았다. 이와 같은 결과를 토대로 다음을 제언하고자 한다. 1. 연구대상자를 확대한 반복연구를 제언한다. 2. 본 연구는 생착이 확인된 환자만을 대상으로 하였으므로, 조혈모세포이식 단계별로 대상자의 희망을 파악하는 추후연구를 제언한다. 3. 조혈모세포이식 환자의 희망을 증진시킬 수 있는 간호중재의 개발을 위한 연구를 제언한다.
Abstract
he purpose of the present study was variables in patients hemopoietic stem cell transplantation(HSCT) for leukemia.A total of 88 participants were recruited at St. Mary's Hospital. The data were gathered from July, 2001 to June, 2002 using questionnaires. Result: The mean score of hope was 3.39±0.35. Positive relationships were found between hope and self-esteem, physician's support, nurse's support, and satisfaction of life. And negative relationships were found between hope and depression, trait-anxiety, and state-anxiety. The major variable, that associated with hope, was the state-anxiety, explained 35.4% in the variance of the hope. Conclusion: Leukemic patients undergoing HSCT tended to have a high level of hope. Higher levels of self-esteem, physician's support, nurse's support and satisfaction of life were related with higher level of hope. And, higher the levels of depression and anxiety were related with a lower level of hope.
