Abstract

One year and three months have passed since the February 5, 2024, announcement concerning the expansion of medical school quotas. However, the events prompting the announcement show no signs of resolution. Views on the subject remain diverse. The government justified the decision to increase the number of medical school admissions, primarily due to the inadequate number of active physicians, which is below the OECD average. An insufficient number of physicians working in rural areas disrupts the seamless healthcare services in those regions. Another reason stated by the government is that fewer residents are applying for essential medical specialties, which results in a further scarcity of specialists in critical care. The final reason for increasing the medical school quota is an anticipated rise in demand for medical services for the older population. However, the Korean Medical Association argues that advances in AI technology, as well as telemedicine and non-face-to-face care, will reduce the workload of doctors and the requirement for additional doctors. Additionally, the governmentʼs estimated medical demand is reported to have been inflated, as it was calculated based on the frequency of use of medical services by the older population. In principle, the medical community has not opposed a hike in medical school quotas. However, if an increase is deemed necessary, they assert that it should proceed only after appropriate preliminary actions. This includes consulting healthcare providers, preparatory measures by medical schools to educate additional students, securing resident quotas for training specialists in essential healthcare fields, and preparation by training hospitals. The medical community insists that any quota increase should be based on a consensus after implementing the above-mentioned policies. Owing to the governmentʼs over-expansion of medical school quotas, there have been resignations by residents and leaves of absence by medical students. This has resulted in the disruption of medical education and the collapse of essential medical services. In particular, residents who worked for low wages and long hours have resigned, and the financial stability of many university hospitals has deteriorated. This has placed them on the brink of deficit and bankruptcy. In response, the government has advanced payment of medical care expenses by three months form the National Health Insurance budget, increased the physicianʼs fees of emergency medicine specialists, and temporarily raised the on-call duty compensation for the night or weekend duty professors. Over the one and quarter year, our government wasted three trillion won of taxpayerʼs money purposelessly. Despite the governmentʼs emphasis that increasing medical school quotas is aimed at addressing the public demand for medical services, and considering that the policy has resulted in medical personnel quitting the field, ultimately, there will be public health risk implications. Furthermore, it has become evident that the policy was misguided, disregarding realistic forecasts of future medical demands and the actual conditions of medical education. Therefore, the priority should be to promptly restore medical school quotas and medical education. Moving forward, decisions regarding the medical personnel should be made through a Medical Workforce Estimation Committee, which must include physicians as stakeholders and ensure sufficient discussion and consensus among all parties involved.
