한국 건강보험통합의 정책실패와그 사회적 파급효과

이 글은 한국 사회보험제도의 하나인 건강보험 통합정책의 실패와 그 사회적파급효과를 학문적으로 규명하는 것을 목적으로 한다. 사회보험제도는 산업생산에참여하는 모든 근로 계층에게 자신들의 근로소득에 비례한 기본생활의 보장을 목적으로 한다. 그것은 자본계층에 계속 집중되는 부를 노동자계급의 주체성과 자율성을 해치지 않으면서 그들에게 재분배시키는 제도적 장치이다. 이는 자본계급이노동계급의 사회보험료 절반을 부담하는 것을 통해서 실현된다. 그러나 한국 정부는 건강보험의 통합을 통해 산업노동과정에 참여하지 않는소외계층에게 현대의 노동계급인 직장가입자집단의 보험료를 재분배시키고 있다. 이것은 사회보험의 재분배 기능을 왜곡하는 것이다. 곧, 통합건강보험체계는 정부에 의한 일종의 근로계층 보험료 착취체계로 전락한다. 국가는 자본주의 생산체계아래에서 다양하게 창출되는 부로부터 거두어들인 일반 조세를 통해 소외 계층의생활을 보장하는 것이 분배적 정의에 부합되기 때문이다. 건강보험통합 이후 유일한 건강보험자인 건강보험관리공단은 보험재정의 기여자인 근로자계층과 고용주계층을 관리·운영 주체에서 완전히 소외시켰다. 이는 막대한 보험 재정 누수 현상을 막거나 예방하지 못하는 결과를 가져왔다. 정부는 이러한 관리운영체계를 통해서 합법을 가장해 광범위한 근로계층의 보험료를 약탈할수 있었다. 곧, 공무원과 사립학교교직원의 정부가 부담해야 할 천문학적 보험료를 체납했다. 그뿐만 아니라 저소득층과 취약계층의 의료보장을 위한 정부의 공공부조재원을 헌법이 규정한 일반조세로 마련하지 않고, 근로자와 고용주계층의 사적재산인 보험료로 대체했다. 그 결과로 근로계층은 매년 인상된 보험료를 내면서도 50% 수준의 낮은 보장혜택이라는 정체의 늪에 빠져 있다. 이들은 가계 빚이 계속 느는데도 사보험 의존을 심화시키는 모순에 노출되어 있다. 이글은 현재 정부가 근로계층의 건강보험료약탈정책을 중단하는 것만으로도 한국의 근로계층은 5.33% 보험료로 100%의료보장의 혜택을 받게 되어 안정적인 기본생활을 영위할 수 있게 된다고 결론내린다.
Abstract
This article attempts to examine the failure of the integration of healthinsurance policy and its social impact in Korea. The social insurance system hasthe goal of securing the basic life of industrial workers in proportion to theirincomes. Thus it is a redistributive system of social wealth that is unequallyaccumulated to the capitalists while not destroying the subjectivity and autonomyof the workers. Capitalists helps achieve such a goal of social insurance bypaying the half expenses of the workers. However, through the integration ofhealth insurance policy, the Korean government gives the benefit of insuranceresources of workers to the alienated class who in fact do not participate in theindustry. This is not fair and a distortion of the redistributive function of socialinsurance. For the redistributive justice of social wealth can be achieved bycaring alienated class through the general tax from various incomes in capitalisticproduction system. After the integration of the health insurances, the National InsuranceServices, which is the sole insurer of Korea, estranged the worker as well as theowners of the industry, who are the real payers of the insurance fees, from themanagement and operation of the insurance system. This results in the massiveloss and failure of stopping the loss of insurance finances. Thus, through thisdisguise of lawful insurance management system, the Korean government isable to despoil the massive sum of insurance money of the workers. That is, themassive amount of insurance fees of the government workers and schoolteachers was not payed. More than that, public assistance resources for the health insurance of the poor and alienated classes, which were supposed to befinanced by general tax, were indeed substituted by the workers’ insurance fees. The result of this is that the more pay and the less coverage of the workers. Workers pay more and more every year but get less and less coverage every yearin return. Therefore, they are in the danger of relying on the private insuranceeven though they are under pressure of serious family debts. Thus, this articlestrongly maintains that, by stopping the Korean government looting insurancepolicy of the workers, the workers can enjoy almost the full coverage benefitwith much less payment.
