이 연구는 서울시 노인 방문건강관리사업을 대상으로 서비스 제공자의 거버넌스 인식을 평가하고, 도시 노인의 방문건강관리서비스의 효과성 제고를 위한 제언을 하고자 하였다. 서울시 5대 권역별로 1개씩의 자치구를 선정하여 보건소 관리자와 동주민센터 방문간호사 총 19명을 대상으로 설문조사와 심층면접을 이용하여 노인 방문건강관리 사업의 거버넌스 평가를 실시하였다. 취약성이 가장 높은 거버넌스 영역은 ‘이해당사자 참여’였고, 자치구 보건소와 동주민센터 방문간호사의 거버넌스 인식 차이가 가장 큰 항목은 ‘일관된 의사결정 구조’와 ‘일관성과 지속성’이었다. 이해당사자 참여의 경우, 주요 이해당사자의 의견을 반영할 수 있는 구조가 미흡하였으며, 지역자원과의 협력을 위한 시스템이 미흡하였다. 또한 일관된 의사결정 구조 측면에서는 이원화된 명령체계에 따른 업무 혼선이 개선의 우선순위로 확인되었다. 추가적으로, 사업의 효과성과 효율성 제고를 위하여 국민건강보험공단 시스템과의 연계, 의료전문가 그룹과의 협력을 통한 방문간호 서비스의 질 제고 등이 제안되었다.
Abstract
This study aimed to evaluate the perception of governance by service providers of home-visiting health services for the elderly in Seoul, Korea, and propose enhancements to increase future service effectiveness. Five autonomous districts representative of Seoul’s five major regions were selected. A mixed-methods approach, combining surveys and in-depth interviews, was used to gather data from 19 participants of managers of public health centers and visiting nurses at community centers. The analysis identified “stakeholder participation” as the most vulnerable dimension. Notably, there was a significant disparity in governance perceptions between public health center managers and visiting nurses, particularly in “coherent decision-making structures” and “consistency and stability.” The study highlighted the lack of effectivemechanisms to incorporate feedback from key stakeholders, such as visiting nurses, and a deficiency in systems for collaborating with local resources. Additionally, work confusion from a dual-command system in decision-making structures has been highlighted as a critical area for improvement. To enhance the efficiency and effectiveness of home-visiting health services, integrating National Health Insurance Service data and fostering collaboration with medical expert groups is recommended. This study underscores the need for robust governance frameworks that include clear decision-making processes and stakeholder engagement to improve elder care services.
