초록

본 연구는 혈액투석환자의 자기관리에 대한 주관적 인식을 파악하고 그 유형별 특성을 이해하고 설명하기 위해Q방법론을적용하였다. P표본은만19세 이상 성인으로 만성신부전을 진단받고 3개월 이상 혈액투석을 하는환자 34명을 편의표집 하였고 43개의 Q표본을각자 의견의 중요도에 따라 강제정상분포가 되도록 9점 척도상에 Q분류하였다. 수집된 자료는 PCQUANL program에 의한 Q-요인분석을 수행하였다. 연구결과, 혈액투석환자의 자기관리에 대한 주관성은 3개의 유형으로 나타났으며 이는 전체변량의 49.9%를 설명하였다. 혈액투석환자의 자기관리에 대한 주관성은 ‘열정적 자기주도형’, ‘비관적 현실위축형’, ‘지지기반 의존형’ 으로 확인되었다. 본 연구결과를 토대로, 환자맞춤형 투석자기관리 교육프로그램의 개발 및 적용을 제언하였다.

Abstract

This study was conducted by applying the Q-methodology, to identified the subjectivity on self-management of patients receiving hemodialysis. A sample of 34 participants was recruited to participate in the study and classified 43 selected Q-samples into a nine-point normal distribution grid. Analysis for collected data were used a PC-QUANL program. The findings of the current study were as follows. The three types account for 49.9% of the total variance: 35.8%, 8.1%, and 6.1%, respectively. Type 1 was ‘enthusiastic self-initiative’, type 2 was ‘pessimistic reality-atrophy’, and type 3 was ‘support-based dependent’. The findings of this study could be able to provide patient-centered and qualified nursing services, and improve satisfaction of the patients who receive hemodialysis treatment by developing customized nursing intervention to enhance self-management of the patients.
