호스피스 돌봄 접근법에 대한 간호대학생의 인식 연구

호스피스 간호는 간호사에게 환자의 신체적⋅정신적⋅영적 요구를 통합적으로 충족시키는 중요한 돌봄 영역이다. 간호대학생의 주관성을 파악하는 것은 학습 요구 파악 및 효과적인 교육 프로그램 개발에 필수적인 과정이다. 본 연구는 Q 방법론을 적용하여 호스피스 돌봄 접근법에 대해 간호대학생 29명을 P 표본으로 조사하였으며, 연구 결과 3개의 유형으로 도출되었다. 유형 1, ‘존엄성 중심의 절제된 돌봄형’은 환자의 존엄성을 최우선으로 고려하며, 신체적 통증 완화와 개별적 신념 존중을 중시하였다. 유형 2, ‘적극적 접근을 통한 편안함 추구형’은 환자의 고통을 신속히 경감하고 편안한 죽음을 제공하기 위해 적극적으로 중재를 수행해야 한다고 보았다. 유형 3, ‘환자 관점 중심의 전인적 접근 추구형’은 환자의 개별적 관점과 정신적⋅영적 지지를 중요시하며, 환자가 자신의 방식대로 죽음을 준비할 수 있도록 지지하는 것을 중시하였다. 본 연구는 간호대학생들의 호스피스 돌봄에 대한 인식이 유형별로 다양함을 확인하였으며, 각 유형의 특성을 반영한 통합 교육 프로그램의 개발 필요성을 제언한다.
Abstract
Hospice care are essential areas of nursing practice that require the integration of physical, psychological, and spiritual needs of patients. Understanding the subjectivity of nursing students is a critical step in identifying their learning needs and developing effective educational programs. This study applied Q methodology to investigate the perceptions of 29 nursing students regarding hospice care, resulting in the identification of three distinct types. Type 1, “Dignity-Centered Restrained Care,” prioritizes the preservation of patient dignity and emphasizes pain relief and respect for individual beliefs. Type 2, “Proactive Comfort-Oriented Care,” focuses on promptly alleviating patient suffering and providing a peaceful death through active nursing interventions. Type 3, “Holistic Individualized Patient Perspective-Oriented Care,” highlights the importance of respecting patients’ unique perspectives, offering emotional and spiritual support, and enabling patients to prepare for death in their own way. This study reveals the diversity in nursing students’ perceptions of hospice care and underscores the need to develop tailored educational programs that address the specific characteristics of each identified type.
