유료 재가 노인 돌봄 노동과 돌봄 관계의 특성

급격한 사회변화로 가족의 돌봄 기능이 축소되고 공적 돌봄 서비스에 대한 수요가 증가하고 있는 현실에서, 이 논문은 유료 재가 노인 돌봄 관계에 놓여있는 돌봄 노동자와 수혜 노인을 한 쌍으로 하여 3쌍, 6명에 대해 채록한 구술사를 토대로, 돌봄 노동의 특성과 재가 돌봄 관계를 구성하고 있는 이해당사자들의 돌봄 욕구의 다양성과 역동성에 대하여 살펴보았다. 노인 돌봄 노동의 특성을 보면 첫째, 수혜자의 돌봄 욕구에 관심을 갖고 충족시키려면 환자의 심리를 잘 파악하고 마음을 맞추기 위해 돌봄 노동자의 감정 조절이 중요한 노동이었다. 둘째, 재가서비스는 노인에게 좋은 돌봄을 제공하기 위해서는 돌봄 욕구의 드러난 부분뿐만 아니라 노인이 살아오면서 채워지지 않은 잠재적 욕구들에 대한 이해와 성찰을 필요로 하는 노동이었다. 셋째, 노인 돌봄은 상호작용을 통한 신뢰관계가 중요한 관계적 노동이었다. 이러한 노인 돌봄 노동의 복합적 성격은 단순한 육체노동으로 간주되어 여성이면 누구나 할 수 있는 저임금 노동으로 평가절하되고 있는 현실에 대해 반박할 수 있는 근거를 제공하고 있다. 유료 재가 노인 돌봄 관계에서 주목해야 할 점은 노인 돌봄 관계가 단순히 돌봄 노동자와 수혜노인의 관계로만 이루어져 있는 것이 아니라 돌봄 노동자와 보호자 가족, 그리고 가족내의 노인과 보호자 가족간에 ‘돌봄 욕구’를 둘러싸고 다층적인 권력관계를 이루고 있으며, 그 안에서 돌봄 욕구의 충족을 둘러싸고 협상과 갈등이 일어나고 있었다. 결론적으로 재가 노인 돌봄의 질을 높이고 노인 부양 가족의 삶의 질을 높이기 위해서는 무엇보다도 돌봄 노동자의 고용조건을 안정화시켜야 하며, 다른 한편 노인의 돌봄 욕구와 분리된 가족 보호자들의 심리적⋅육체적인 어려움과 돌봄 욕구를 인정하고 사회적으로 지원할 필요가 있을 것이다.
Abstract
With the diminishing role of family for elderly care, the demand for services and workers engaging in public elderly home care are growing very fast. This paper is a trial to find out the best context for matching both the satisfaction of home care givers and receivers. Care workers could deliver good services for the people in need of help in a condition they could have relational autonomy in their own job. In this paper, the author tries to interpret the meaning of care work based on the in-depth life history interviews about 3 pairs in elderly home care relationships. First, care work can be characterized by mind services providing their intensive concern on care receivers. Care givers has to be prepared to control their emotions in order to understand the psychological state of patients and provide them best customized personal care services. This can be accomplished by contextual knowledge acquired by frequent personal exchanges and interactions. Second, care workers are expected to be equipped both with manifest and latent needs of elderly people, which are not fulfilled with their lives. Third, care work is a relational labor, in which trust relationship between service providers and elderly are playing pivotal role. The meaningful trust between two can be developed by frequent and sincere personal interactions. Care relations are also situated in asymmetrical power relations between care workers and receivers. Care receivers, their family carers, and care workers are all engaging in the politics of needs interpretation on what is the social nature of care work, how the need of the elderly people is estimated and identified, and how far care workers extend their services for the people in need of care. In order to find a possible solution for this problem, a public debate on the nature of care work including social, economic, and moral meaning of care work has to be activated. Through public discussions both on the autonomy of care workers and the rights of choice of elderly people and their family, our community could find out more concrete and realistic measures to meet the expectations of those people concerned.
