가족요양보호사와 가족돌봄자의 돌봄부담 영향요인 비교 연구

Characteristics of Certified Family Care Providers(gajokyoyangbohosa) and Informal Caregivers, and Predictors of Their Caregiving Burden

초록

본 연구의 목적은 2008년부터 시행되고 있는 노인장기요양보험제도 안에서 파생된 ‘가족요양보호사(가족돌봄자가 공식적으로 요양보호사로 지정되어 노인을 돌보는 경우를 지칭)’와 비공식 가족돌봄자 두 집단 특성을 비교하고, 부양부담에 영향을 미치는 요인에 두 집단간 차이가 있는지를 비교하는 것이다. 이를 위해 서울 및 경기지역에 거주하며 노인장기요양보험제도의 재가서비스를 제공하는 가족요양보호사 85명과 제도를 이용하지 않는 가족돌봄자 150명을 대상으로 기술통계 분석을 실시하여 인구사회학적 특성, 요보호노인의 건강상태, 돌봄부담 등에 있어 두 집단의 차이를 살펴보았다. 또한, 회귀방정식 모형을 통해 가족요양보호사와 가족돌봄자의 돌봄부담에 영향을 미치는 요인을 파악하였다. 연구 결과, 가족요양보호사는 가족돌봄자에 비해 여성이 많고, 월평균 가구소득 수준이 낮은 것으로 나타났다. 또한, 가족요양보호사들이 돌보는 노인의 ADL 제한 정도와 문제행동은 가족돌봄자들이돌보는 노인의 건강상태에 비해 훨씬 더 열악한 것으로 나타났다. 회귀분석 결과, 가족요양보호사의 돌봄부담에 영향을 미치는 요인과 가족돌봄자의 돌봄부담에 영향을 미치는 요인(주돌봄자의 건강상태, 요보호노인의 문제행동, 다른 가족의 도움정도, 요보호노인과의 친밀감)은대체로 유사한 것으로 나타났다. 그러나, 가족요양보호사의 경우 월평균 가구소득이, 가족돌봄자의 경우 주돌봄자의 연령, 요보호인과의 관계, 요보호노인의 ADL 제한정도가 돌봄부담에 영향을 미치는 것으로 나타났다. 이러한 결과는 노인장기요양보험제도 내 가족요양보호사와 관련된 쟁점을 논의하는데 필요한 기초자료로 활용될 수 있을 것으로 기대된다.

Abstract

The purposes of this study were to investigate the characteristics of certified family care providers (“gajokyoyangbohosa”) participating in the Korean Long-Term Care Insurance (LTCI) system and examine whether the predictors of caregiving burden differ between those of gajokyoyangbohosa, who receive financial compensation as official care providers under the Korean LTCI system, and those of informal family caregivers. Using data from a 2011 survey (N=235) conducted by the Korean Women’s Development Institute, descriptive analysis including chi-square tests and t-test were conducted together with multiple regression. The results showed that compared to informal family caregivers, gajokyoyangbohosa were predominantly women, had lower levels of socioeconomic status and labor-force participation and their care-recipients reported poorer health conditions and more severe functional limitations. The shared predictors of caregiving burden for the two groups included caregiver’s health, care-recipient problematic behavior, level of social support from other family members,and level of affection that the caregiver reported for the care recipient. Differences in predictors of caregiving burden between the two groups were as follows: Level of monthly household income predicted the level of caregiving burden among gajokyoyangbohosa, while caregiver’s age, relationship with the care-recipient and the care-recipient’s ADLs limitations predicted that of informal family caregivers. The findings of this study suggest essential resources for the further development of the Korean LTCI system.
