아동기 역경경험과 성인기 우울의 관계에 대한 연구

Associations between adverse childhood experiences and adult depression

초록

본 연구의 목적은 아동기 역경경험 실태 및 역경을 구성하는 총 13가지 세부 유형의 개별 및 누적 영향이 성인기 우울에 미치는 실증적 영향력을 검증하여 사회복지적 개입의 단초를 제공하는데 있다. 이를 위해 2017년 한국보건사회연구원에서 실시한 ‘아동가족 생애경험 실태조사’에서 성인 4,008명 대상으로 수집한 횡단데이터를 활용하여 로지스틱 회귀분석을 실시하였다. 본 연구의 주요 결과는 다음과 같다. 첫째, 한 가지 이상의 역경경험에 노출된 연구대상자는 전체의 80.4%(n=3,224)에 육박하였고, 다섯 가지 이상은 27.1%(n=1,085)로 분석되었다. 그리고 가장 유경험자의 비중이 높은 유형은 ‘집단폭력피해 및 목격’ 49.6%(n=1,987)인 반면, 가장 적은 유형은 ‘가구원 수감’ 0.8%(n=8)로 나타났다. 둘째, 아동기 역경경험의 누적에 따른 성인기 우울 위험비는 1개 1.85배에서 5개 이상 3.08배로 나타나 누적 증가 양상을 보였다. 셋째, 세부유형에 따른 성인기 우울 위험은 ‘신체방임’이 3.39배로 가장 높았고, 그 다음은 ‘정서방임’ 2.25배, ‘또래폭력’ 1.35배의 순서로 검증되었다. 이와 같은 연구결과를 바탕으로 아동기 역경 발생 예방 및 성인기 우울에 대한 조기개입을 위한 정책 및 학교사회복지 실천 현장 중심의 실천적 제언을 실시하였다.

Abstract

The current study aims to explore the prevalence of adverse childhood experiences (ACEs) consisting of 13 different types and examine the association between ACEs and adult depression. 4,008 cases of cross-sectional data from ‘A study on life experiences of children and families’, which was conducted by Korea Institute for Health and Social Affairs in 2017. The mains results are as follows. First, 80.4% (n=3,224) of the sample were exposed to at least one type of ACEs, and those who reported exposure to more than five types accounted for 27.1% (n=1,085). Second, analysis revealed that there was a cumulative increase tendency in adult depression risk from 1.08 times of one type exposure to 3.08 times of more than five types. Third, physical neglect was strongest predictor of adult depression with 3.39 times, followed by emotional neglect 2.25 times, school violence 1.35 times. Based on the results, political preventive strategies of ACEs and clinical implications for early intervention in adult depression were suggested.
